Montana’s Crib Program
Program Information
Safe Sleep for Baby provides Pack ’n’ Play Playards and other safe sleep materials at no cost to
families in need. When a family is identified, local public health departments or early childhood
specialists make a referral to the program. This program ensures that a safe sleep environment is
available for Montana infants. Education and training is provided to parents about the importance
of safe sleep in an effort to reduce the risk of Sudden Infant Death Syndrome (SIDS).
The primary source of funding for Safe sleep for Baby comes from the Healthy Montana Teen
parenting program, therefore the majority of parents that receive resources from safe sleep for
baby must be between the ages of 15 and 24.
If a client is outside of these age parameters, we still might be able to provide a crib for your
client. Please call or email Healthy Mothers, Healthy Babies-MT to confirm the availability of
funds prior to submitting the order form.

Instructions:
Step 1:
Public Health Nurses or other family support organizations can order a Pack-n-Play Playard by
filling out the Safe Sleep for Baby order form.
Step 2:
Submit the order form either via email or paper mail to:

Step 3:
Deliver the pack-n-play to the client. When delivering the crib, the program requires that an
appropriate professional from the requesting organization demonstrate how to set up the Pack-NPlay and go over the educational materials included with the safe sleep kit. After the clients
have been taught how to properly use the crib and the educational materials have been presented,
the program release form will need to be signed by the client and your programs witness
(typically the professional that is delivering the crib).
Step 4:
Submit the release form to Healthy Mothers, Healthy Babies-MT.
Step 5:
Follow up three months later- have the client fill out the Three Month Follow up form and then
have the form submitted to Healthy Mothers, Healthy Babies. If this is not possible, please still
submit the form with the clients name and the reasons the form was unable to be filled out.

Orders are made weekly on Tuesdays and Fridays.
Shipping time:
Cribs can take anywhere from 1 week to 3 weeks to arrive at your facility!
It may be in the best interest of your organization to keep an “emergency” crib on hand for
clients that need a crib immediately. If you choose to keep an extra crib on hand, the release form
will need to be submitted when a client has been identified. An order from is still required for the
extra cribs with a note on the order form that designates it as an “emergency” crib. Once the
extra crib has been matched with a client please submit an order for another “emergency” crib.
