U14 – U17 OSA Crusaders Summer Select Rosters 2016
Congratulations to all who tried out and made the 2016 OSA Crusaders Summer Select GIRLS basketball
teams. Please note the following information regarding accepting your spot, registration and payment
deadlines, uniforms, first practice and parent/player meeting, and team rosters.
TEAM ROSTERS – Team rosters with assigned jersey numbers are listed on the following page.
PROCESS FOR ACCEPTING YOUR SPOT - You will have until 5pm on TUESDAY, MARCH 8TH to accept or
decline your spot.
• To accept your team placement: You MUST email [email protected] with your
player first and last name, specific team name and confirmation of acceptance. For example – I, Jenny
Hooper, accept my spot on the U15 Girls National team.
• To decline your team placement: Please email [email protected] with your player
first and last name, specific team name and confirmation of declination. For example – I, Jenny Hooper,
decline my spot on the U15 Girls National team.
PLEASE NOTE: After you send your email to accept/decline your spot, you will be sent an automatic emailed
response, which will contain REGISTRATION LINKS. All players must register (either online or via the hard
copy registration form on the last two pages of this document) by Friday, March 11TH.
FEE STRUCTURE - All fees include weekly practices, bi-weekly skill academies, and all spring and summer
tournaments. Fees DO NOT include the summer uniform fees as explained below.

UNIFORM PROCEDURES / URGENT DEADLINES
Wednesday, MARCH 9TH through Sunday, MARCH 20TH - Uniform Orders Due
• We will be wearing the same jerseys and shorts this summer as we did last summer and the same
jerseys as we wore this past fall/winter season (OSA summer shorts are different than the fall/winter
shorts, so please be sure you order the Summer OSA Crusader shorts if you donʼt have them already).
ALL participants MUST wear the OSA jersey and OSA Crusader shorts to play this summer. Shorts
are $30 and Jerseys are $45, plus tax and a $4 handling fee from Under Armour. Sample sizes will be
available at OSA beginning March 9th if needed. Due to the limited amount of time between tryouts and
our first tournaments, players will have a limited amount of time to order! It is imperative that you donʼt
miss these order dates!
• To Order Uniforms: A link will be emailed to all those who make a team roster on Wednesday, March
9th. The online store will only be open from MARCH 9TH– 20TH ONLY, so please make this a priority so
your athlete is ready to play on their first tournament weekend! Players MUST order their assigned
jersey numbers (found on team rosters).
FIRST PRACTICE and PARENT/PLAYER MEETING – All U14 – U17 teams will have their first practice on
Wednesday, March 9th from 7:45 – 8:45pm at OSA, with a Parent/Player Meeting from 8:45pm – 9:00pm.

By committing to participate in the OSA Crusaders Summer Select Program, I understand that:
• All team fees must be paid in full or according to the payment schedule set by Omaha Sports Academy. I am
responsible for all fees by said due dates, regardless of how much my child attends or if my child quits playing
mid-season.
• Any default payment may result in my child being dismissed from the team until my account is brought current.
• Any delinquent fees will be turned over to a collection agency if I fail to keep my arrangements.
• All payments are non-refundable.
• I understand that if my child played on an OSA team the previous year, it does not necessarily mean he/she will
play on the same team or at the same level this year.
• I will not be verbally abusive, rude, disrespectful or sarcastic to players, coaches, officials, other fans, and OSA
Management, or to the management of leagues and tournaments that we attend.
• Disregard for appropriate behavior may result in my child being dismissed from the team.
• Players perform better when a lot of positive cheering carries throughout the game regardless of the score or
past performance.
