Applying for an Initial
Passport
You will need:

☐ your proof of citizenship (e.g., birth
certificate); this will be returned1
☐ your new photo DO NOT ATTACH
☐ identification and photocopy of same
(e.g., driver’s license)2: both sides on 8
½” x 11” sheet of white paper

in an envelope and take it to USPS or UPS so
that you can get a tracking number.
*Additional fee for first-time processing
You will need to present yourself with
your documents at one of the agencies on
the reverse side. We suggest a post office
so that you can mail your application at
the same place. Call first to find out if
there are specific hours for passports. You
will sign your application in front of the
U.S. government representative. This
person will look at your ID and return the
original.

Completing the application online
Go to:
https://pptform.state.gov/FraudAbuseNotice.aspx
And click on the Submit button at the bottom of the
“Complete Form Online” section. After you
complete the form, you must print it out. Then
follow the instructions above.

need to go to a Post Office or UPS to mail your
application. Be sure to ask to send it certified,
which will provide you with a tracking
number. This way, you can be sure that your
application has been received. It may cost a
few dollars more, but it’s worth it because you
have your original documents in that
envelope.
