Applying for an Initial Passport You will need: ☐ completed application (DS-11) DO NOT SIGN ☐ your proof of citizenship (e.g., birth certificate); this will be returned1 ☐ your new photo DO NOT ATTACH ☐ identification and photocopy of same (e.g., driver’s license)2: both sides on 8 ½” x 11” sheet of white paper ☐ and a check made out to “Department of State” Application Fees Passport Book: $110+ $25* Passport Card: $30 + $25 Both: $140+ $25 Renewing a Passport Put your: ☐ completed application (DS-82) ☐ expired passport ☐ new photo ☐ check made out to “Department of State” Application Fees Passport Book: Passport Card: Both: $110 $30 $140 in an envelope and take it to USPS or UPS so that you can get a tracking number. *Additional fee for first-time processing You will need to present yourself with your documents at one of the agencies on the reverse side. We suggest a post office so that you can mail your application at the same place. Call first to find out if there are specific hours for passports. You will sign your application in front of the U.S. government representative.
