The Justin Slade Youth Foundation was established as a result of the tragic passing of Justin Slade on November
5th, 2004. Justin had a passion for life and a desire to help others and his legacy to the community was brought
forth through the opening of The Justin Slade Youth Foundation, the center opened April 1st, 2006 at the Haxton
Center. The goal of The Justin Slade Youth Foundation is to empower the youth of our community.
Please join us for our 11th Annual Justin Slade Youth Foundation Golf Tournament, Dinner and Silent Auction
June 23rd, 2017. We are currently seeking donations for Tournament Prizes and Silent Auction Prizes. The
Tournament is currently looking for Major sponsors for the dinner and event. All proceeds from the tournament
will go to The Justin Slade Youth Foundation to assist with programming for the Youth of our Community. To

(includes 18 holes of golf, a golf shirt for each participant, a golf cart and dinner). If you would like to sponsor a
hole, it is $1500 (with this you get a team in the tournament), or if you would like to sponsor a hole without a team,
its $750.
