Enrollment Application Bark of the Town's initial doggie introduction session begins with a temperament and behavioral evaluation by our trained staff. This generally requires a half day or more as each new member is gradually introduced to the regular attending pack. The success of this evaluation is based on each dog's comfort level and how quickly they adapt to our pack. Obviously, there should be no history of aggressive behavior, each member be up-to-date on all vaccinations, and all dogs over 6 months of age must be spayed or neutered to attend. Not every dog is successful at daycare integration. If after several attempts of pack introduction and we observe anxiety, fear-aggression or snapping, we will let you know if your dog does not “pass”. After all, if it's not fun for your dog than what's the point! Please bring in the completed form with the following information completed with your dog's first visit and we can expedite your introduction. All dogs must be non-aggressive and in good general health. Owners should be prepared to provide copies of current yearly vaccinations for DHLPP, Rabies, and Bordatella (Kennel Cough). We recommend Bordatella vaccinations every 6-months. Puppies should be at least 12 weeks old with their second round of vaccinations (and a reasonable size) before entering puppy areas. After 6 months of age we will require spay/neuter based on each breed. Each member of appropriate age should be on a monthly flea/tick preventative, such as Frontline or K9 Advantix. One-time flea applications, flea collars, herbal or natural approaches are not effective in a daycare environment. Members attending should possess a collar with ID tags and a leash when entering and leaving our facility. Oh... and be ready to play, have fun, and make new friends. PLEASE NOTE: Every dog is evaluated on their merit and behavior in a pack atmosphere. They may do well in a family situation at home but in a pack atmosphere they can become anxious and withdrawn, resulting sometimes in aggressive behavior which does not make a good time for anyone. Understanding dog breeds, behavior and pack mentality is the key to creating a safe and fun dog daycare atmosphere. Please do not be offended if we deny your dog as our decision is based on their safety and the safety of our pack. Bark of the Town takes safety very seriously. 1020 N.W. 67th Street ● Oklahoma City, OK 73116 ● (405) 842-1515 Owner Information Primary Owner Name_______________________________________________________________ Address______________________________________City____________________ State ____________________ Zip Code__________________ Home Phone(____)_____________Work(____)______________Cell(____)_____________ Email________________________________________________________________ Secondary Owner Name_______________________________________________________________ Address______________________________________City____________________ State ____________________ Zip Code__________________ Home Phone(____)_____________Work(____)______________Cell(____)_____________ Email________________________________________________________________ Emergency Contact Name_______________________________________________________________ Home Phone(____)_____________Work(____)______________Cell(____)_____________ Email________________________________________________________________ 1020 N.W. 67th Street ● Oklahoma City, OK 73116 ● (405) 842-1515 Pet Information Dog's Name____________________________Breed_______________________________ Color____________________Weight__________________ Male____ Female _____ Age _____ Birth Date __________________________ Neutered: Y/N Spayed: Y/N Veterinary Information Veterinarian's Name____________________________________________________ Veterinarian's Phone Number ____________________________________________ Vet's Address (if known)_________________________________________________ Vaccination Dates: Last Physical Exam_____________________________________ DHLPP (or the equivalent) ____________________ Corona: ___________________ Rabies Vacc ________________________ Tag #____________________________ Bordatella ________________________________ (Must be current within 6 months) Flea/Heartworm Preventative_____________________________________________ Medication Name_______________________Frequency____________Quantity_____________ Name_______________________Frequency____________Quantity_____________ Name_______________________Frequency____________Quantity_____________ 1020 N.W. 67th Street ● Oklahoma City, OK 73116 ● (405) 842-1515 Additional Information How long have you owned your dog?_______________________________________ Is he/she crated or kenneled during the day?
