PERMIT TO TAKE OUT, MOVE AND TRANSPORT ROCK MATERIAL General Guidelines for submission through BLESS Application through BLESS is only for NEW Applications. Renewals and amendments to the current licence are currently not available. The applicant is required to attach the necessary documents as listed below in the specific guidelines and is to be submitted together with the completed application form for submissions through the BLESS system. The applicant can upload into the BLESS system all necessary supporting documents in Word, Excel and PowerPoint format using the upload facility provided in the BLESS system. The applicant can scan original documents and these scanned images can be uploaded into the system in jpeg format or other relevant formats. In the event that the applicant is unable to upload certain documents (drawings), please submit manually to the relevant processing centres. For manual submissions, the applicant must quote the BLESS submission number in each and every document. If any fees are required by the agency, the applicant may utilise the E-Payment function in BLESS provided the said agency has activated this function. If any payment is to be made, a payment request from the agency shall be prompted to the applicant. Alternatively, the agency can request that payment be made manually. All information from Suruhanjaya Syarikat Malaysia (SSM) related to the company is made available in BLESS, thus the applicant is no longer required to furnish this information and documents. Specific Checklist and Guidelines 1. Form 4 – Permit Application. 2. Project Site Master Plan. 3. A copy of ownership land official search (if related). 4. Written Permission from landlord, if the application involving ownership land and the applicant is not the landlord. 5. Written Permission from Enforcement Officer, if the application is on reserve land. 6. Lorry registration number. 7. Application payment RM100.00. 8. Roadways and transfer location site plan. 9. Contractor appointment letter. 10. Company profile. 11. Environmental Management Report – refer to Laporan Pengurusan Alam Sekitar technical checklist by Bahagian Alam Sekitar, Perbadanan Putrajaya. 12. Planning Permission approval letter by Bahagian Perancang Bandar, Perbadanan Putrajaya (if related). 13. Confirmation of land quantity issued by Consultant Engineer / Architect. Additional Checklist Application of Permit to Take Out, Move and Transport Rock Material 1. Every application of permit to take out, move and transport rock material should be made in Form 4 in this method. 2. When the permit of taking out, move and transport rock material from the Federation's land or reserve land is being issued, deposit as much as thirty percent of fee amount imposed on the permit must be paid in pledge of due performance and compliance any condition the licence or permit is subjected to. 3. Deposit mentioned in sub-method (2) can be forfeited either overall or a part of right which is decided by Government. 4. When the application is approved, and after annual fee and deposit payment, if applicable, permit issuance in Form 4c Code’s should be done by Commissioner. Application of Permit to Move Rock Material 1. Every application for permit to move rock material should be made in Form 4c Code. 2. When a permit to move rock material application agreement and payment fee that been fixed in method 28 made, Land Commissioner should issued permit in Form 4c Code. Fee of Permit to Take Out, Move and Transport Rock Material Fee to be paid for permit to take out, move and transport rock material should be as the following: Details Rock Material Type Fee to be Paid (1) Soil RM2.00 per metre cube (2) Laterite RM2.00 per metre cube (3) Loam RM2.00 per metre cube (4) Clay RM2.00 per metre cube (5) Sand RM2.00 per metre cube (6) Grass patch RM2.00 per metre cube (7) Rock, stone or granite in any appearance RM2.00 per metre cube (8) Marble RM2.00 per metre cube (9) Cement RM2.00 per metre cube (10) Brick RM2.00 per 1000 unit (11) Tile RM2.00 per 1000 unit (12) Any rock material that does not allocated other than RM3.00 per metre cube that (Unsuitable Material) (13) Any form of completed manufacturing goods from rock RM10.00 per 100 unit material that does not allocate other than that With condition that Government could, in which particular case, exempt overall or part of payment fee that is fixed under this method.
