Thank you for enrolling in our program. This is our FINAL INFORMATION SHEET
for the camp (listed above) which provides all the important details of which you need to
be aware (below). This sheet, along with the MEDICAL FORM can be printed from
your Registration Confirmation email with the link provided, along with being
available on our Korn Camp website (under camp forms). Please remember to print
these documents.

Medical Form
The form can be printed from your Registration Confirmation email (it was attached). You can
also access on our website (www.mitchkorn.com) under the CAMP FORMS tab. DO NOT MAIL
IT BACK. Bring it with you totally completed to Check-In on the first morning. Players cannot
skate without it!
Lunches
The Arena will coordinate a GREAT daily lunch for the goalies at a reasonable cost. We will either
send a Lunch sign up via e-mail prior to camp, or you will be able to sign up at Saturday morning
Check-In.
Balance of Fees
Your Registration/Confirmation acceptance was e-mailed to you. Most applicants pay a Tuition
Deposit ($100) to register. The Balance of Fees remain. All Balance of Fees along with a 5%
“registration fee on the full tuition” will be charged to the credit card on file on May 6. All fees are in
U.S. funds. Any change in credit card account numbers can be revised by logging into the On-Line
Registration site and your account which was established when you registered.

All cancellations must be made via a phone call or e-mail directly to Mitch Korn.
All camp tuition (deposit and balance of fees) is accompanied by a 5% “Registration Fee” charged
by the on-line service to cover credit card and administrative costs. All “Registration Fees”
and your $100 tuition deposit are non-refundable & non-transferable.
All “balance of fees” ($495 balance) will be refunded up to 3 weeks (21 days) prior to the day
camp begins with a written doctor’s medical excuse. After that point, 80% of these “balance
of fees” will be refunded with notification prior to the 7 DAYS BEFORE camp begins.
There is no refund of any fees should a player drop out of camp for any reason within the 7
days before camp begins.
Should a player be asked to leave camp by the Director for any reason, no refund is made.
All refunds will be credited back to the family credit card used to register.
Locker rooms
We expect all players to RESPECT this beautiful facility, the staff and the other participants. For
females participating in the program, separate locker room facilities will be available. All players
should be able to dress themselves. Parents, please refrain from entering the locker rooms…space
is very tight. Parental assistance is allowed outside the locker rooms, only.
Equipment
Remember, all goalies must have and properly wear, full, legal equipment. We will provide the onice jersey sponsored by CCM. For the off-ice block, all goalies need sweats/shorts and sneakers
along with dry socks, t-shirt and undergarments. Please bring at least 1 change of garments.
BONUS…Extra Ice Time:
Each day 4 goalies are chosen for “extra ice time” to tend net for the “Defense Program” during the
goalie’s lunch break. Those chosen will eat lunch after the 1.5 hr. ice session.
The Final Day
We will have a ceremony on the final day of camp. All the parents are welcome. The ceremony is
tentatively scheduled for 5:15pm. Awards will be presented.

We will work hard to ensure that all participants have a fantastic experience. We expect all players
to be willing and able to participate in all our activities, to follow the rules established, to have a good
attitude and sense of humor, work hard, and to display camaraderie and sportsmanship. You only
get out of this program what you put in!
