As part of the ongoing Code Administrator training, reviewed COCAG Appendix C “Procedures for
Code Holder Exit” which will include a process to address LRNs not belong to the code holder. This
will be effective 10/21/16.

There were 3 CIC assignments made in September
There were no 5XX-NXX assignments made in September
There were 5 CIC reclamations/returns in September
555 Reclamation as of 9/30/16
o 1727 letters have been sent via US Mail with 475 being returned as undeliverable

497 outlining steps necessary to activate the new 838 NPA overlay

INC Activities

Issue 825 – Update the COCAG Appendix C – Procedures for Code Holder Exit, to address a
returned NXX with an LRN that does not belong to the code holder. Issue was accepted at INC 150
and went to initial closure. Guidelines were updated 10/21/2016.
