“Try It… It’s Good for Life” Game Loan Expectations Form The “Try It” game is suitable for activities and events for parents and/ or children ages 3 to 12 years promoting awareness and knowledge of healthy eating and the benefits of physical activity. As a borrower of the game you are responsible for ensuring that it is returned in the condition that you receive it. The following measures need to be taken to maintain the game: Prior to assembling, complete the “before use column” on the Checklist Form. A supply of forms is included in the “Using the Try It …it’s Good for Life Game” folder which should be found in the smallest carry bag. The game is relatively easy to assemble but does require following directions carefully that are also in the folder. During its use, the loose parts should only be attached if the game is supervised to prevent loss of these pieces. Positioning the game near a staffed reception area would be appropriate supervision. After disassembly, complete the “after use column” on your Checklist and fax to Julie Cloutier at (705) 721-1495. If any pieces are missing during Checklist completion, please contact Julie Cloutier at (705) 721-7520 ext 7337 or via email [email protected] as soon as possible. If you have any questions regarding its use, availability or assembly, please contact Julie. Arrange for return of the game as outlined below.
