JOB PROFILE VARIOUS TITLE: SOCIAL WORKER CENTRALIZED SERVICES HUB CLASSIFICATION: SOCIAL PROGRAM OFFICER 24 The Centralized Services Hub Social Worker reports to a Team Leader. The Team Leader reports to an excluded Manager responsible for Centralized Services Hub. JOB OVERVIEW The Social Worker at the Centralized Services Hub will work closely in a team environment being responsible for the collection and assessment of information about prospective caregivers across MCFD service lines, with the goal of supporting local offices in the initial assessment of caregivers. The position will require; cross program understanding of the caregiver approval process, work with prospective caregivers to ensure the appropriate assessment information has been completed and received by MCFD, document information related to assessments, and liaises with the local offices about the outcome. As noted above this position will provide support across MCFD program areas, and will support the local offices across the province. ACCOUNTABILITIES 1. Appropriate Resources: a) receiving requests from district office social workers with respect to planning for children and youth being brought into care on an emergency and planned basis b) searching out and coordinating placements for children and youth being discharged from a resource c) coordinating assessment material for out-of-region placements if required 2. Liaises with district offices regarding placements of children and youth in care by: a) attending internal social work meetings on a regular basis to discuss and report on specific issues and general outstanding resource issues b) acting as information broker for children needing placement and/or service and transferring information to the appropriate resource worker c) providing ongoing evaluation and assessment of a child’s and youth’s progress during placement to assist in the planning process 3. Facilitates the provision of foster home resources by children-in-care by: a) b) c) d) recruiting, orienting, studying, recommending for approval and training foster parents monitoring and provision of service and supporting MCFD caregivers coordinating the negotiation of special foster home rates, as required liaising with local MCFD caregiver associations and regional councils and/or MCFD staff to resolve issues and provide information of mutual interest e) may be assigned to negotiate special care homes and/or parent contracts 4. Recruits and develops contracted resources by: a) consulting with line child welfare workers and supervisors to identify resource development needs Career Group: Job Family: Social Services Adoptions/Guardianship Job Stream: Role: Revised Date: Professional October 2015 b) assisting the contract negotiator in defining the best way to meet identified needs c) recruiting and screening potential care givers d) gathering and assessing all available information about prospective MCFD caregivers to inform child placement suitability. e) providing background information to the negotiator to aid in contract negotiation f) interpreting and explaining the contractor relationship to service providers and ministry staff and maintaining that relationship in all dealings with service providers g) reviewing and understanding the contract negotiated, and interpreting same as required 5. Monitors the provision of service by: a) evaluating the resource to ensure program is being provided as outlined in the contract b) setting overall goals with the resource for ongoing service delivery c) ensuring that review and revision of individual client goals is completed by involving the client’s child welfare worker d) coordinating all admissions and discharges and attending related meetings e) providing ongoing feedback to the resource regarding quality of service f) providing information to the negotiator to assist in contract renewal, as to whether service is acceptable g) maintaining up-to-date records and statistics on resource referrals and occupancy 6. Supports MCFD caregivers and contracted resource providers by: a) b) c) d) interpreting ministry policy and requirements to service providers in a positive manner representing the needs of service providers to ministry staff in a positive manner acting to resolve problems between service providers and Ministry staff ensuring that requirements for use of resources are clarified, understood and met 7. Reporting on allegations of child abuse and neglect in child welfare resources according to regional and provincial protocols. 8. Performs other related duties such as: a) exercising spending authority as appropriate; i.e. approving homemakers for foster homes b) carrying out special projects as required. c) Works respectfully and collaboratively with Aboriginal communities and other cultural communities to support children, youth and families. JOB REQUIREMENTS MSW; or M.Ed. Counselling/MA Clinical Psychology 2 years (preferred 3 or more years) related experience; or BSW; or BA in Child and Youth Care; and 3 years (preferred 4 or more years) related experience; If your degree was obtained outside of Canada, you need to confirm it has been assessed for equivalency through the International Credential Evaluation Services (ICES).
