CSE-IT Quick Start Guide
Accessing the CSE-IT Online
In order to access the CSE-IT online, you must first fill out the Service Provider Information
Form at: http://goo.gl/YVSkOF
After completing the SPIF you will be provided the link to the CSE-IT. Please bookmark this
link or save it somewhere you will remember. You can also write down the link here:

Filling out the CSE-IT
Where the form asks you for the following, input the information as indicated:
Agency Name: Enter your agency name, including county if you are a county agency.
Service Provider ID: Your employee number or name. Use the same information you
entered on the SPIF every time you fill out the CSE-IT.
Youth Code: Youth's birth year + last four digits of case id
Example Youth Code for Tom Grant, born 2001, MR#396542
Youth code: 20016542
When you finish answering all questions on the CSE-IT, you will be directed to a summary
page. You may print or save a PDF of your responses from this page. You must hit the final
“Next” button on the summary page for your responses to be submitted.
Screening Guidelines
The CSE-IT is designed to be used with all youth ages 10 and up and should be administered
every 6 months. It is suggested you administer the tool when a youth returns from a period of
being away from care.
