We have aimed to make the system as user-friendly as possible. This guide outlines how to use the system and complete each section of the report. Key points You will receive an invitation by email Everyone listed as a DAIP contact with the Access and Inclusion team will receive an invitation to the online reporting system. Please let us know of any changes to contacts. Keep it secure Each email invitation is unique and acts as a key to your report. Anyone you forward it to will be able to edit the report. Only one invitation can be sent per email address, so keep it in a place where you will remember it. Start working now, save your work, and come back later When you click ‘Next’ on each page your work will be saved. You will be able to close the browser window and resume your report later by clicking on the link in the original email invitation. You can print out your report Toward the end of the report, you will be able to view your responses on one page. You can print this page or copy the text into a document for further editing. This is intended to help with your approval process. Lodge the report when it is ready Once you click ‘Done’ on the final page you will not be able to make any further changes. 2 Accessing the online reporting system If you are listed as a DAIP contact for your organisation, you will be sent an email invitation to the online reporting system. The email will come from ‘[email protected] via surveymonkey.com’. If you do not receive an email invitation, please contact [email protected] to request one. Scroll to the bottom of the email and click ‘Begin your DAIP report’. This link will take you to SurveyMonkey. You can save your work by clicking ‘Next’ on any page. After you close the page, you can resume by opening the original email invitation and clicking on the ‘Begin your DAIP report’ link again. Please note: Your invitation is the key to your report. If you forward it to anyone else, they can open and edit your report. Only one invitation can be created per email address, so make sure you keep it where you will remember it. If you lose your invitation, you will have to send an alternative email address to the Access and Inclusion team. A replacement invitation will be sent to that address and can then forwarded on to you. 3 Entering your details Fill in all the fields and click ‘Next’. After you click ‘Next’ it is recommended that you close and open the page again from your email invitation. Click ‘Prev’ to check that your details are still there. 4 Reporting progress on each DAIP outcome There is one page for each outcome area. Begin by filling in the number of strategies that were planned. Then, out of that number, take the ones that were implemented and categorise them by how effective they were. For example, if your organisation planned 12 strategies for Outcome 1, your report might look like this: 5 Next, describe what your organisation achieved in support of the outcome. Put in as much detail as you can. Unfortunately some formatting, such as paragraphs and bold text, cannot be saved. Finally, you can upload photos to illustrate your activities. You can upload one image file for each outcome area, or if you have a series of photographs for one outcome area, upload a document file with the photographs inside. The limit is 16MB per outcome area. Captions and photo descriptions can be provided in the text box for the outcome area, in the file name, or in the file itself. The Commission may contact you about featuring a photo in the Minister for Disability Services’ yearly report to Parliament. However photos of people cannot be featured without written permission from the person or their guardian. A permission form is available on the Commission's DAIP reporting web page, and can be uploaded on the same page as the picture. 6 Agents and contractors To complete this section you will need information about agents and contractors who have interacted with the public on behalf of your organisation. First, describe how your organisation informed those agents and contractors about its DAIP. You can tick all the boxes or none. Next, describe how agents and contractors informed you about what they did to help implement your DAIP. You can tick both boxes or none. 7 Finally, describe any specific activities agents and contractors undertook to implement or comply with your DAIP. This is another opportunity to highlight what your organisation has achieved. 8 Challenges in implementing your DAIP If you wish, you can provide information about challenges you faced in implementing your DAIP. You can leave this section blank if you wish. You can also write a detailed comment if you wish. 9 Displaying your report This page gives you an opportunity to print out your report and seek internal approval and/or save it for your records. You can print the page by clicking File > Print or by pressing Control-P. Another option is to select and copy the text, then paste it into a Word document for further editing. This option will be required if you want to apply formatting to the text responses. 10 Feedback On the feedback page you can give feedback on the Access and Inclusion team, the DAIP reporting system and the Commission’s online DAIP resources. Lodging your report Once your report is approved and ready for lodgement, click ‘Yes’ and ‘Done’ to lodge it. Note that once you click ‘Done’ you will not be able to return and make further changes. If your report has not been completed or approved yet, you can stop here and return later. If you need confirmation that your report has been received, please contact the Access and Inclusion team. 11 Troubleshooting I don’t know how to access the online system. If you are listed as the DAIP officer for your organisation, you will be sent an email invitation to the online reporting system. The email will come from ‘[email protected] via surveymonkey.com’. If you did not receive an email invitation, please contact [email protected] to request one. The link does not open. Check your internet connection (e.g. by opening a different page). Confirm that SurveyMonkey is not blocked on your internet connection. I tried to resume my report but the information I previously entered is missing. Find the original email invitation that was sent to you via SurveyMonkey, open it and click ‘Begin your DAIP report’. This should bring you back to where you left off. If this doesn’t work, contact the Access and Inclusion Team for help. SurveyMonkey says I have already completed the survey. You may have accidentally clicked ‘Done’ on the last page. If you forwarded your invitation to someone else, they may have accessed the report and clicked ‘Done’. Please contact the Access and Inclusion Team for help. I think I lodged my report but I’m not 100% sure it has been received. Contact the Access and Inclusion Team for confirmation. I lodged my report but there was a mistake in it. If there is a factual error in the report please contact the Access and Inclusion Team, stating the change needed. The team will edit the report manually for you. I lodged my report but forgot to save a copy, and now I need one. Please ask the Access and Inclusion Team to email you a copy of your report. Allow a few days for your request to be processed. 12
