*By signing above, I, the team contact, agree that I have read, understand, and agree to comply with the
Information for Team Entries as well as the Ladies Summer Hockey League Rules & Regulations. I also
agree to ensure that each player/coach/bench staff is aware of these rules, and completes the Team List
before participating.
All games are at Westwood Arenas on Wednesday’s from May 10, 2017 to August 16, 2017. Start times
for the games are between 7:45 PM and 10:00 PM. Total team registration is $4500.
Please be reminded that your $1500 deposit must be received no later than March 15th in order to
secure your spot in the 2017 Ladies Summer Hockey League, Wednesday summer season.
To register, send this form along with your deposit in the amount of $1500, payable to “Ladies Summer
Hockey League”, to 24 Greenbriar Road, Brampton, ON. L6S 1V6. Your deposit cheque may be post-dated
up until April 1st 2017. Additional payments of $1500 each are due on May 10th and June 14th respectively.
