Phase I: Excel Tracking Tool+ (Option 1)
• Teams will begin by tracking their behavioral health &
health measures in an Excel-Based Patient Registry.
• One Team Member will be responsible for managing
the Excel Patient Registry.

• The Registry will have the same type of Dashboard
metaphor for reviewing an individual case or group of cases.

• There are modules for organizations with E.H.R.s
(dbMotion Agent) and those still on paper records
(dbMotion Collaborate)
