Service description
My Place has been operating as a provider of individualised and flexible supports to people
with disability and their families since it was established by the founders of local area coordination in 1996. My Place now supports more than 250 people with disability to live in their
own homes or remain in their family home and become valued and contributing members of
their community. My Place does not provide any group home or congregate care services. The
individualised arrangements that are most commonly utilised are described below. What
connects all the arrangements is that the person is always in control of what supports they
receive; when, where and how they receive those services; and from whom they receive those
supports. Naturally, people are free to choose any mix of arrangements that better suits them
at any time they wish.
Self Management – As the name suggests, this gives you total control over, and responsibility
for, your own funds and supports.
Shared Management – Most people who use My Place prefer the Shared Management
arrangement, where you still manage your own services and employ or contract your own
support people. The difference from Self Management, however, is that you have My Place
administer the funds. In other words, you design and run your own service.
Provider Management – A number of people with disability and their families prefer a support
arrangement that leaves them with the least administrative hassle or legal responsibility. If you
prefer this type of arrangement, you can ask My Place to coordinate your services and employ
your support people on your behalf.
Pricing information
My Place has a number of support arrangements from which you can choose. Depending on
the management approach you choose coupled with the support arrangement My Place will
provide a price that is tailored to that arrangement. Please contact My Place for specific

My Place provides reasonable and necessary disability related supports
and services to eligible individuals through contract with the Disability
Services Commission. Individuals and their family or other stakeholders
make independent decisions about which supports or services they wish
to purchase and this guide is provided to support that process.
