VOLUNTEER DRIVER
This form and documentation of license and insurance must be in the school office at least two days
before the field trip. This form is not transferable to other family members.
Evidence of Safe Environment approval must be on file in the school office at least two days before
the field trip.
Drivers must provide copies of:
DRIVER LICENSE – Drivers must be over 21 years old and have a current Oklahoma Driver License
INSURANCE - Drivers must meet requirements as set out in Oklahoma State Statutes, including:
1. All vehicles used to transport students must be insured for $10,000.00 for bodily injury per
person and $20,000.00 bodily injury per accident.
2. For round-trips of 250 miles or longer, a minimum coverage of $50,000.00 per person and
$100,000.00 per accident is required. (A higher amount up to $300,000.00 is recommended)
3. In case of an accident, the vehicle owner’s insurance is primary.
I AFFIRM THAT:
 My vehicle is covered by insurance sufficient to meet the requirements in 1 below.
 Every passenger will be individually seated, wearing a seat belt, or in a car seat approved by
Oklahoma State Statutes.
 I am over the age of 21 and have a valid Oklahoma driver license.
 I will not make any stops nor deviate from the planned field trip route.
 I will not purchase nor allow purchase by students under my supervision of any unauthorized
items (souvenirs, treats, games, etc.) during the field trip.
 I will use my cell phone ONLY for an emergency.
 I will supervise the students in my charge during this field trip.
