Registration & Residency Information Sheet
DOCUMENTATION TO VERIFY YOUR CHILD’S AGE
Please provide documentation from one of the following that applies to your
child’s place of birth:
If your child was born in the U.S:
Child’s FULL SIZE birth certificate (original copy)
If your child was NOT born in the U.S:
Child’s passport or permanent resident alien card.
If a child resides with a legal guardian other than the parent:
The legal guardian must provide:
Valid Photo ID.
Child’s FULL SIZE birth certificate (original copy)
State of Connecticut issued documentation that they
are the student’s legal guardian.
(Notarized letters from the biological parent is NOT acceptable).

If the parent or legal guardian is not a West Hartford resident but is staying
with a West Hartford resident, then both parties must sign a Certification of
Residency and both parties must provide:
Valid Photo ID
West Hartford resident must provide their proof of residency.
