PALMETTO TEEN GROUP APPLICATION
2016-2017 School Year
The Palmetto Teen Group is open to homeschooled students who are in the 6th grade (11 years old by
September 1, 2016) through the 12th grade. To join the teen group, send your completed application and
membership dues as instructed below. Select if your child will participate in the Middle or High school group, not
both. The membership dues are $20 per family per year. In order to be a part of PTG, member families are
REQUIRED to actively participate through volunteering.* All group communications will be by email via the Yahoo!
group. The Yahoo! group membership will be purged on September 30th each year. The deadline for membership
this year is January 31, 2017 - no exceptions. Print a copy of this form and complete it.

volunteering.* To be successful PTG needs members to volunteer to organize events, to help manage
events (by providing help to the organizer) and to chaperone events. Below is a suggestion of some of
