CFC - GOTSOCCER INDIVIDUAL PLAYER REGISTRATION
If you have multiple children playing for Continental FC you are entitled to a registration
discount. Have your team manager contact Susan Hennelly (Club Administrator) at
[email protected] PRIOR to registering to receive the required discount code.
1.

2. The parent/guardian will have three (3) choices:
a.
Family Registration (NEW ACCOUNT) – anyone with two (2) or more children playing in
Continental FC can have a family account
b. Individual Users – NEW ACCOUNT
c. Individual Users – EXISTING ACCOUNT (Most players have an account but do not know.
They can click on the “Player Password Lookup” link to retrieve the information. The new
U09 (currently U08) teams will not have any information.)
3. Click on ‘Continue’ once all information is updated and valid.
4. Select Your Team (VERY IMPORTANT)
a.
Please communicate to your team the NAME, YEAR and GENDER (ex - Continental FC
Barca ‘00M).
b. Team age groups in GotSoccer are based on TODAY which is 2012-13 Age Groups (not
the new 2013-14 Age Groups).
c. This is software and the new season does not officially begin until August 1st. On
August 1, 2013, the teams will automatically move to the new age groups.
d. When registering for the 2013-14 season, you MUST select the age group according to
the 2012-13 season.

Payment Voucher # - This will only apply to those families with multiple children playing for CFC.
a.
Managers – If you know anyone on your team with multiple children playing in the
club, you must contact me prior to them registering. I will send a Payment Voucher # to
you, the manager, to provide to the family registering. NO DISCOUNT will be given after
registration is complete.
b. If you don’t have multiple children playing, leave blank.

Grade – Use the current grade (School Year 2012-13).

Complete all other information.

10. Complete Emergency Information.
11. Complete Medical Information.
12. Complete Physician Information.
13. Complete Insurance Information.
14. Click ‘Continue’.
15. 2013-14 Club Fees should be checked.
16. Click ‘Continue’ (If applicant will be submitting a scholarship application, check the box.)
17. Complete payment by credit card.
18. Review information.
19. Enter your full name.
20. Check box agreeing to the items.
21. Click ‘Finish & Submit Application’.
22. On the next screen, each parent/player MUST PRINT the MEDICAL RELEASE FORM, SIGN IT and
return to the TEAM MANAGER.
23. Copy both sides of your insurance card and give to the manager.
24. Copy your birth certificate and email or give it to your manager.
25. Email a digital COLOR photo to your team manager.
