Mapping-Training-201.. - Health Federation of Philadelphia

Data hygiene for better reports and searches
June 2016
Goals
 Why to map
 What to map
 When to map it
 How to map it
Why do we map?
 Helps you understand the data path
 Where was it entered?
 How can I pull it into reports?
 Helps you build more efficient workflows
 You’ve noticed a trend you can’t explain
 Maintain data quality
Why do we map?
 I thought I did this already?!
 You did… but the data (or reporting needs) are
constantly changing:






Labs
Medications
Workflow and documentation processes
System upgrades
CPT codes
UDS measures
What to map?









Labs
Medications
CPT and other codes
Procedures
Problems
Visit types
Immunizations
Allergies
Custom mapping
 educations, goals, and other profile items
When to map it?
 Before each monthly quality report run:
 Labs
 Medications
 When you add a new visit type or procedure that
relates to an existing report or measure:
 CPT codes
 Procedures
 Visit types
 When a new measure is announced
 All the data that are relevant (you won’t always know, but you can
start)
How to map labs:
 Where should I map labs?
 Setup – Tracking – Lab Tracking
Labs, cont.:
 Lab Tracking – map lab tests AND values (where
appropriate)
Labs, cont.:
 CD4 count
How to map Linked Visits:
 Setup – Linked Visit Type
 Add CPT and/or dx codes
 Linked visit types can be deleted/fixed via a ticket to i2i
How to map medications:
 Some medications will auto map but you will most likely
need to add some
 Do not use Medication Categories
 Setup – Patient Profile Item – Medications
 1st check Medications to see if the Medication or
Medication Group is listed

If not, click Add and enter the name – click ok – click close
Medications, cont.:
 Medication Mappings


Click the medication you want to map and click Edit
Click Add and search for the medications to map
Medication cleanup
How to map a procedure/referral:
 Setup – Patient Profile Items – Procedure/Referral
 Check to see if Procedure/Referral exists, if not – Click
Add

Add linked visit types and/or schedules
Procedure/Referral, cont.:
 Mapping in multiple places
 Various types of data (CPT, dx,
lab, etc.) for the same data point
 Example: Cervical Cancer
Screening
 Lab/Linked Visit Type/Procedure
Immunizations:
 Setup – Patient Profile Items – Immunizations
 Use for immunizations outside of the childhood immunization
schedule
 No need to use Vaccine Type Mappings: it will create duplicate
entries in your i2i immunization tables

Should be automatically mapped through your interface
 Linked Visit Type/Vaccine Type/Schedule
Allergies:
 Use to ensure the “right” patients are in the denominator
 patients who are allergic should not count against you!
Custom Mapping:
 Treatment Plans
 Other – map in CDI
 Set value

List/number/date
 Examples:

Hospitalization

Screening questions (PHQ, GAD)
Smoking Status

How do I choose where to map
something?:
 Depends on what you are trying to do AND where your
data are



Set a schedule (or use as a criterion)
 Procedure/Referral
 Problem
Set your denominator
 Linked visit type
 Problem
Account for different types of results (date, list, number) or
non-standardized results
 Other Profile Item
 Labs
Data Maintenance:
 Marking test patients in i2iTracks
 Patients – Patient Info – lookup test patient

next to status – click check box
Data Maintenance:
 CDI maintenance – 1 to 2 times/week
