Application Workshops
Three orientation workshops for potential applicants will be held to learn more about the One Nation
Bay Area Project and how applications will be evaluated. Organizations planning to submit an
application are strongly encouraged to attend. An RSVP is required. We recommend that you attend
the workshop in the region where you plan on implementing your project.

Once in Grantee Center, you can sign in if you already have a user name and password. If you are a new
applicant to The San Francisco Foundation select "create a new user account."
Only online applications for funding will be accepted. Online applications must be submitted by 5:00p.m.
on December 9, 2011 to The San Francisco Foundation. Please note: applications will not be accepted by
mail or other form of delivery.

One Nation Bay Area Project Application
Your application does not guarantee a grant. A review committee from all of the participating institutions
in the One Nation Bay Area Project will jointly review the grants and make recommendations for funding.
Follow the application format described below, responding in the space provided. Please limit your
responses to no more than 650 characters per question. If you have any questions, please do not hesitate
to contact us.
Part I: Project Description
1. What is the principal objective of your project? In this section, please include a description of which
community need(s) your project will address. How was this objective selected? Describe the extent to
which key partners and constituents were involved in defining the principal objective.
2. What constituency will your project serve? Please include information on the age range, ethnicity,
religious affiliation, income level and specific geographical area of community members served by your
project.

3. Who is responsible for project leadership? Please describe—and include background information—on
the individual(s) or committee in the congregation, organization or collaborative that will be responsible for
oversight of the project. Please include contact information including email addresses for the project
leaders.
4. What partner organizations/ congregations (if any) are involved in the project’s implementation?
Please discuss the level of commitment partner organizations have made to the project in terms of time
and space for leadership, funding, congregational participation, etc. Describe, if appropriate, the history of
these partnerships and provide one or two examples of past accomplishments or collaborative projects.

5. What is the project and anticipated time frame? Please describe the project, the steps involved in the
project, and the dates by which you hope to accomplish each step.
6. What specific goals are you trying to achieve? Please list the specific measurable goals you have set for
your project. For example, a coalition of organizations, neighborhood groups, etc. may have a goal that
states, “Our coalition will work in partnership with at least 15 Muslim leaders and residents in our
neighborhood to serve 50 meals per week to low-income individuals in our area.”

7. What is the level of community involvement or engagement? Please describe the level of community
engagement of both American Muslim and non-Muslim community members in your project, including
community leaders, faith leaders, government officials, business or school staff, and/or other individuals
from the wider community. How long have you been working together? Are there other collaborative
projects that you have developed that we should be aware of that have had meaningful community
impact?
8. How does this project further the goals/mission of the organization and/or collaborative?

9. If your proposal were to receive funding from the community foundation, does it have the potential to
become self-sustaining when the community foundation’s support ends? If so, how?
10. If your proposal were to receive funding, how will you know if your project achieved its goals? Please
describe how you plan to evaluate your project.
