steadilyontherisethepastfewyearsandwethankallofyouforyourcontinuedsupportin

TheCalgaryOldTimeHockeyPlayersAssociationawardsannualscholarshipstostudentswhoare
attendingsecondaryeducationalinstitutions.Inorderforastudenttoreceivesuchanawardthe
followingcriteriamustbeaddressed:
1. Thecandidatesmustbeattendinganaccreditedsecondaryeducationalinstitution.
2. Thecandidatesmustsubmitatranscriptoftheirlastsemestermarks.
3. Aletterofreferencefromateacherorguidancecounselor.
4. Anoutlineoftheirpastandpresentcommunityandathleticinvolvement.
5. Abriefoutlineoftheirfutureplans.
6. StudentIDnumberandSINnumber.
7. Homemailingaddress,postalcodeandtelephonenumber.
8. Forasonordaughterofamembertoapply,themembermusthavebelongedtotheassociation
foraminimumoftwoyearsandbeanactivevolunteerinassociationevents.
9. Successfulapplicantscanre-applyeverytwoyears.

Thisscholarshipisawardedannuallytoastudentwhoisintheprocessoffurtheringhis/her
education.
2. COTHPASCHOLARSHIPS(sponsoredbytheCOTHPA)
Thesescholarshipsareawardedannuallytoastudentwhoisintheprocessoffurthering
his/hereducation. 3. COTHPASCHOLARSHIPS;(FINANCIALNEED)(sponsoredbytheCOTHPA)
FromtimetotimetheCOTHPAmayawardascholarshiptoastudentwhodoesnotqualifyfor
oneoftheabovescholarshipsbutwhichthecommitteefeelsa"specialneed"isrequired.
Applicationdeadlineisfastapproaching.Allscholarshipapplicationsmustbereceivedby
August31st,2016.

$125 - Members in Good Standing
$150 - Non-Members (Golf registration comes with a 1 year membership in COTHPA)
Entry fee includes 18 holes of golf, cart, driving range, dinner and prizes.
Register as a team of two or as an individual.
Tournament format: 2-person best ball format will be utilized.
