I hereby give consent for my child (named above) to play for the OLQP Falcons Soccer Club (hereinafter referred to
as “The Club”) in an age group and team as appointed by The Club Committee.
I agree to abide by the constitution and by-laws of The Club and those of the NSW Churches Football Association
and any other association The Club may play with (hereinafter referred to as “The Associations”)
In the event that my child is injured at an event organised by The Club or The Associations, permission is hereby
granted to send my child to the nearest doctor or hospital.
I further agree that in the event of an injury to my child, I shall indemnify and keep indemnified The Club and its
office bearers against and from all and any claims by myself or by any other person in relation thereto.
I acknowledge that The Club has full discretion as to whether to offer my child a playing position and to determine
the team and that registration is subject to and conditional upon the payment of the full fees prior to the due date as
determined by The Club.
I acknowledge that The Club shall not refund any monies paid other than in the event that The Club is unable to
place the above named child in a team.
I acknowledge that I have read The Clubs Code of Conduct to my child and we both agree to abide by the code and
to ensure family and friends at the games adhere to it.
I hereby give permission for photographic images to be taken and used in any of The Club’s media (website,
newsletters, Social Media, etc.)
I understand that loss or damage to the jersey allocated to my child (except playing wear and tear) will result in a $80
fee payable by me to The Club by the end of the current season.
I acknowledge that The Club is operated by parents and guardians of the children who play in The Club and that all
parents and guardians of children who play in The Club accept responsibility to assist The Club in its operations as
requested by The Club Committee. Failure to assist The Club will result in increased registration fees. Substitution or
delegation of responsibilities to children is not accepted.
I acknowledge that the parents or guardians of my child will be required to assist in The Club’s canteen
during the season. Failure for a team to turn up or not have enough people at their scheduled time may result
in a ‘fine’ payable by the team to The Club to cover the canteen’s potential loss of income due to the team’s
action.

Registration Fees are an essential part in keeping the club running. The fees go towards a number of things including but not limited
to: The Churches’ Associations fees, training equipment, Training Uniforms and grounds keeping.
A jersey will be allocated to each player prior to the first game and you will be responsible for the care of this jersey during the
season. The jersey remains the property of The Club and must be returned to The Club at the end of the season. If the jersey is lost or
damaged (except for normal wear and tear) the parent/s will pay The Club an $80.00 fine.

Training
All teams are required to attend training on week days (evening) and for the duration of the season. Each player must attend training
except for circumstances beyond their control. Once the teams are formed, parents of each team will negotiate and agree on a
training evening/s and time/s.
Parents/Guardians must remain with their children during training sessions and games to support them and be there in case of
accidents etc.
Training Uniforms received as part of the registration must be worn during all training session.

Play by the rules and understand the spirit of the game.
All players and spectators must respect the referees, association referees whether they are official or unofficial.
All players and spectators must respect the players and the spectators of the opposing teams.
All players, officials and spectators to conduct themselves in a manner which will not bring The Club or themselves into
disrepute.
Report any injuries as soon as possible to a team manager, coach or club official.
Report any incident of abuse of the code to a club official immediately.
Be on time for training and games.
Sometimes the answers are not always as simple as you might think. Club officials, Managers and Coaches are volunteers.
NO ONE IN THE CLUB RECEIVES PAYMENT. If you have a legitimate concern, approach a club official.
Team Officials can not cancel/forfeit a game unless directed by The Club’s President. 48 Hrs notice must be given to The
Club’s Committee before the game. Fine of $100.00 will apply and payable to the Club by the team if notice is not given.
Failure to uphold the “Code” will lead to disciplinary action from The Club
