Athletic Participation Form (including insurance
information)
Birth Certificate
Proof of Residency (not a cell phone bill) June, 2017

Sportsware (Emergency Contact form – online)
Concussion testing completed prior to tryouts
All paperwork must be submitted at the same time

Online paperwork through Synergy is supposed
to be starting July 1, 2017
Once we have that up and running, we will
advertise and post directions on the Howard HS
website
Most of the paperwork will be completed online,
but hard copies, or digital copies (uploaded) will
still be required for Physicals, Proof of
Residence, and Birth Certificates
Academic Eligibility
A student must comply with the Academic
Eligibility for Extracurricular Activities
Policy(Policy #9070). Students must have
earned a 2.0 (weighted) GPA with no
more than 1 failing mark (E) during the
previous quarter.
The grade entered on the report card
determines eligibility, not a subsequent
change (except in the case of teacher error).
Drug and Alcohol Policy
All students must comply with the Drug and
Alcohol Policy(#9230). Use and/or
possession, intent to distribute and/or
distribution of drugs/alcohol/non-controlled
substances is in violation of this policy and
will result in a 30 school day suspension
from extracurricular activities. A copy of
the policy is located in the school office.
This includes the sharing of prescription medications
