Nominations for all award categories may be individual submission
or by nomination. Each award is based on calendar year.
A USPTA professional-level member residing in the Midwest Division since June 2013 may apply
for one or more of the awards. Nominee must be a current certified professional member of the
USPTA Midwest Division in good standing. Recreational Coaches may apply with Professionallevel members for the Lesson for Life award for either under or over $10,000. Facility Manager
does not have to be USPTA member.

The award will be divided into two subcategories to
honor the various ways USPTA professionals
contribute to charities in their area, events
raising less than $10,000 and events raising more
than $10,000. An event may win the award only
once in a particular category.

career are very important.
3. Include a color photograph and biographical information.
4. Individuals may submit scrapbooks that include this application. To
receive National credit for the nomination, a division must include the
completed application in its Division notebook, as well.
We encourage applicants to be pre pared to attend the
USPTA Midwest Division Conference if they receive the award.
Players of the Year
Men’s or Wo men ’s
Open, 35 and over, 45 and Over, or 55 and Over
1. Tournament results must be listed in the order of priority (i.e., USPTA
tournaments are given highest preference), as indicated in the application.
Scores are not necessary.
2. USPTA Players of the Year must be certified members in good standing. All
other member categories are ineligible, including honorary members (except
where they were a certified member before being awarded honorary
membership) and applicants.
3. Player of the Year award categories are: Men’s Open Division, Men’s 35 and
Over, Men’s 45 and Over, Women’s Open Division, Women’s 35 and Over and
Women’s 45 and Over.
4. Key tournament results are those during the award period (July 1 to June 30).
Previous years’ results and rankings should be submitted.
5. Include a color photograph and biographical information with the application.
6. Attach any support materials, such as newspaper clippings, interviews,
scrapbooks or career highlights of the applicant’s career.
7. Division nominations may not include supplemental scrapbooks supporting
this award.
