You can fill out this survey electronically or by requesting a paper survey from the main office at your child’s
school. The handwritten instructions can be found at the end of this document.
Filling out the Survey Electronically
In order to enter your survey, enter the URL in your browser http://sayyespsps.com/.
Once there, you will see Figure 1.

A - Select your survey language. Available language options include: English, Spanish, Burmese and
Vietnamese.
B - Click “Complete Survey” to begin the next step of providing your consent.
Providing Your Parental/Guardian Consent
Once you have clicked on the link above, you will come to a page requesting your consent prior to
completing a Family Survey.

A - Read the SYTE Survey Informed Consent and click the check box if you would like to consent –
remember that your consent is voluntary.
B - Click “Take Survey” to begin Family Survey.

A1 - Read the introduction and click yes or no to opt your child into the Postsecondary Planning System.
• If you click “Yes,” you will be prompted to provide your first and last name as well as your child’s
first and last name.

B - Click “Next” to begin the Family Survey.
C - As you proceed throughout the Family Survey, you will be able to monitor your percent complete.
• Please note, you cannot save or come back to an incomplete survey. Please be prepared to
devote an appropriate amount of time to complete the Family Survey in its entirety.
A2 - Read the introduction and click yes or no to opt your child into the Postsecondary Planning System.
• If you click “No” and “next” you will be forwarded to a “Consent Withheld” page and you will not
proceed to the Family Survey.

Student Information Instructions
After providing your consent, you will proceed to a Student Information page where will verify that you are
completing a survey for a valid Guilford County Schools student.

A – From the “School Name” drop down menu, select your child’s school. Only the initial 12 launch schools
will appear.
B – From the “Grade Level” drop down menu, select your child’s grade. Grades K-12 will appear.
C – Enter your child’s Student ID Number. You will be asked to verify this number again after entering to
ensure that the ID provided matches. If you need assistance finding your child’s Student ID number, please
contact the data manager at your child’s school.
D – Click “Next” to proceed to the Family Survey.
Filling Out the Survey
At the bottom of every screen, you will see a progress bar and have the ability to navigate backwards and
forwards throughout the survey. If you miss a question, you will be shown an error message, and asked to
answer the missing questions before being allowed successfully move forward with the Next button.

Answer all questions to the best of your knowledge and click next at the end of each page. There is only 1
question on the first page. Then answer questions 2-19 and click next. Then answer questions 20-22 and
click next. Then answer questions 23-24 and click next. Then answer questions 25-26 and click next. Then
answer questions 27-32 and click next. Then answer questions 33-34 and click next. Then answer
questions 35-38 and click next. Finally, please answer questions 39 which is a text box. Once you have
completed the survey, click “Next”
After you click submit, you will see the screen in figure 8.

You will be asked if you have another child in your household that you need to a fill out a survey for. If you
do, you click on the link labeled, “Return to Say Yes Guilford” and you will be sent back to the initial survey
page, which is on page 1.
Whether you fill out one or more surveys for your household, your results will automatically be sent to Say
Yes Guilford.

Filling Out a Paper Survey
HANDWRITTEN INSTRUCTIONS
1. If you choose to fill out the survey using a paper copy, you can use a pen or pencil.
2. The paper survey is available in Burmese, English, French, Spanish and Vietnamese. Please contact
your child’s school for the language needed.
3. You will need to have your child’s GCS Student ID Number which is the same number as your child’s
lunch number and is between 5 and 10 digits long. If you need assistance finding your child’s
Student ID number, please contact the data manager at your child’s school. The number can be
found on any official school document (report card).
4. Fill in the Student ID Number on the top right and then fill in the bubble that corresponds to that
number under it. Please fill in the bubble completely.
5. Answer questions 1 – 25 to the best of your ability. Please fill in only one bubble per question.
6. When you are finished, please return the survey with your child to school and have them give it to
their homeroom teacher or their Say Yes Family Support Specialist.
7. Please fill out and return one survey for each child in your household.
***If you indicated a need for support services for your child on any of the questions you answered, please
contact the Say Yes Family Support Specialist at your child’s school.***
