Relay For Life of Milan T-Shirt Order Form
Encourage your Team Members to become a member of the
Hope Club!
Team Members who become Hope Club members during the
2016 Relay Season raise $100 on an individual basis or as a
team average.
 This will be based on the Team Captain’s discretion.
 Team Captains also have the choice of not placing a shirt
order for their team, thus cutting down on our event
expenses.
 T-shirt information is NOT pulled from the Relay website,
you must turn in this order form to get shirts for you and your
team. If all team members registered online are going to
qualify for the Hope Club, we can pull a list from the website,
but we must be informed of this.
 All team members must be registered for the event
(online or scannable form) BEFORE ORDERING a t-shirt.
 All participant and Team Captain t-shirts can be picked up
on Bank Night on August 23rd. Survivor shirts are available
the day of the event.

We have had issues with people not registering and we have been left with too many extra tshirts. Therefore, if you are not REGISTERED by July 12th you will NOT be getting a shirt.
