Evaluation Method: Choose Paper or Online. (Courses taught online must choose online evaluation
method.)
Distribute FUSA Results to FUSA: Select whether to send the results of the 5 FUSA questions (numbered 4852) to FUSA. Please note that students will answer these 5 questions on every survey.
Delivery Method:
If you choose to survey students online, please select one of three delivery methods:
o URL Only – the unique course URL is distributed from the IDEA center to faculty and enables (in fact
requires) the faculty member to share the URL with students. With this method, faculty control exactly
when students have access to the survey.
o Email – the unique course URL is emailed directly to students on the date that the survey window opens
and students may log in to complete the survey at their convenience. This method involves a reminder
email with the link every 5 days, until students complete their survey or the survey window ends.
o Both – the URL is distributed to faculty member via email AND students receive email reminders with
the link.
If you choose to survey students using paper surveys, select the Paper option. A packet will be delivered to
your campus mailbox.
o
Paper – the evaluation results are collected on Paper forms
Long or Short Form:
You may select either the long form survey or the short form. The IDEA long form provides student
evaluation data to guide pedagogical improvement and professional growth. The IDEA short form might be
a good option for professors who’ve taught the same course for a long period of time and simply want to
check to be sure they remain effective.
