Greetings!
Below please find information regarding our Gala Cash Raffle fundraising initiative held in conjunction with our Annual Gala Dinner
Dance — to be held Friday, June 2, 2017. Proceeds from this year’s event will benefit St. Joseph’s Health’s Cardiovascular Center.
The odds of winning are very good as only 300 tickets will be sold and all of the prizes are CASH!
Tickets are $100 each, and three winning raffle tickets will be drawn for prizes of $10,000, $5,000 and $2,500. Winning tickets will be
drawn on Monday, June 5, 2017, at the Foundation office. You do not need to be present to win.
If you wish to purchase a Cash Raffle Ticket(s), simply fill out the “mock” ticket below and return to Pamela Kleine, Foundation
office, along with payment. We will then send you the actual numbered raffle ticket.

WHAT IS THE ST. JOSEPH’S HEALTH
FOUNDATION CASH RAFFLE?
HOW DO WE CL AIM OUR PRIZE?
The St. Joseph’s Health Foundation Cash Raffle is a fundraiser
consisting of the sale of 300 tickets at $100 each. Three
winning raffle tickets will be drawn for prizes of $10,000,
$5,000 and $2,500.
All parties on the ticket must provide full names and addresses
for the proceeds to be distributed. Once the ticket holder(s)
have been notified, a date and time will be established for the
disbursement of the prize money and the W-9 IRS form.
WHEN WILL THE DRAWING BE HELD?
IS THERE AN AGE LIMITATION?
The winning tickets will be drawn on Monday, June 5, 2017,
at the Foundation office.
Yes, the raffle is only open to persons eighteen (18) years of
age and older.
DO I HAVE TO BE PRESENT TO WIN?
No, you do not need to be present to win.
HOW WILL I BE NOTIFIED IF I AM NOT PRESENT?
The person whose name is listed first on the ticket stub will be
contacted the Monday following the Gala. If there is more than
one individual listed, it will be up to that person to contact the
remaining purchasers.
A cash raffle ticket is void, and the holder thereof is ineligible to receive any prize relating to
it, if there is reasonable proof, in the sole discretion of St. Joseph’s Health Foundation, that
the ticket was stolen, not properly issued, ineligible, illegible, altered, counterfeit or printed in
duplicate or in error.
By participating in this cash raffle, each participant and his or her successors, heirs and assigns,
releases and forever discharges St. Joseph’s Health Foundation, and affiliates, members,
employees, directors, officers and agents from any and all costs, fines, claims, losses, damages,
or liabilities of any nature resulting directly or indirectly from acceptance, receipt, ownership,
possession, misuse or use of any prize or participation in this cash raffle.
