To increase blood flow, rub your hands together for 10 to 15 seconds. Tear open Alcohol Prep Pads. Clean ring finger and let dry. 1. CHIP Blood Collection Instructions 2. CHIP Lab Request Form (CLRF) 3. Blood Sample Return Bag (oxygen scrubber and desiccant are sealed inside Bag – DO NOT REMOVE from Bag) 4. AdvanceDX100 Serum Separator card (ADX card) 5. Two (2) Lancets (one is an extra) 6. Sterile Gauze Pad 7. Alcohol Prep Pad 8. Sterile Bandage 9. Pre-addressed and Pre-paid Mailing Envelope 3 – Nick Your Finger Remove the protective tip from the Lancet. Twist cap and pull. Place the tip of the Lancet slightly off center on ring finger. Hold Lancet steady, press down firmly until it “clicks,” then release. 1 – Fill Out Your Information You need to have fasted for 12 hours prior to collecting your blood sample. Drink water only. ADX Card Place ADX card on a clean, dry surface. PRINT your name and date on the ADX card. Complete the CHIP Lab Request Form. 4 – Drip Blood on Card (1 of 2) Squeeze finger from the base upward to increase blood flow. You will place blood drips in the box on the left of the ADX card. Squeeze finger until the blood drops freely, 4 to 5 drops. Standing also helps the blood flow. Flip Over 5 – Drip Blood on Card (2 of 2) Four (4) LARGE drops will usually be enough to reach the 1st line. IMPORTANT – Without touching the card (touching reduces the size of the blood drop) CONTINUOUSLY apply 4 drops of blood to the card. Allow the blood to drop on the card. DO NOT stop and wait between drops. 8 – Mail Sample to the Lab 6 – Allow Blood to Dry for 30 Minutes Use Sterile Gauze Pad to wipe excess blood from your finger and apply Sterile Bandage. Allow the blood on ADX card to migrate, stop moving and then air dry for 30 minutes. 7 – Preparing Sample to Ship Tear open the Blood Sample Return Bag at notches. Insert ONLY the ADX card into the Blood Sample Return Bag and seal (DO NOT REMOVE the oxygen scrubber and desiccant). 9 – Accessing Blood Results (1 of 2) Locate the return envelope included in your CHIP Lab Kit. Expect three (3) to five (5) business days from the time you mail the envelope until your results are ready to view. Write your return address on the envelope. To view your results visit http://www.bayshorelabs.com and Click on Patient Log In. 10 – Accessing Blood Results (2 of 2) In the Client Number field, type in 20146. In the Medical Record # field, type in the following: 20146 + Last 4 Digits of your Social Security # + Your Date of Birth (DOB). For example, if this was your information: • Last 4 Digits of Social Security #: 5037 • Date of Birth: November 2, 1960 20146 You would enter into the Medical Record # 201465037110260 field the following: 20146 5037 110260 5037 Place Blood Sample Return Bag and CHIP Lab Request Form into envelope and seal. Drop in mailbox. End Client Last 4 # of SS# DOB In the Password field, enter the last 4 digits of your Social Security #. Then click the Login button. SEE YOUR RESULTS!
