How to Take Healthy Checklist Quiz on
Let’s Move! Child Care Website
1. Go to the Let’s Move! Child Care website, healthykidshealthyfuture.org
2. Click on “It All Starts with You: Sign Up Now” on left hand side
3. Under “Step 1: Sign Up and Receive a Certificate for Participation,” click “sign up now.”
4. Enter your information to complete sign-up process
5. When sign-up process is completed, you will see a page with a link to “Take the Checklist
Quiz.” To proceed, click this link. (To come back and take checklist quiz later, click on
“Take Our Healthy Checklist Quiz” tab on left hand side of home page.)
6. To begin taking quiz, read about the 2 parts of the Checklist Quiz, then click next.
7. Enter email address, and click next.
8. Read instructions, click that you have read the instructions, and click next.
9. Complete the quiz.
10. After completing checklist quiz, you will see a screen telling you that “you are now ready
to create your action plan.” Any items in which you are not meeting best practice will be
listed under “items” on the left side of the screen. Choose 2-3 of these items, and drag
them one-by-one to the “Priorities” box. Click Next when done.
11. You should now see a screen telling you it is time to create your action plan. Make sure
you have about 15-30 minutes to complete this action plan. You can come back to it later
if you do not have the time after finishing quiz, but it must be finished within 2 weeks. If
you are ready to make action plan now, click Next. You may contact CCR&R Healthy

you would like help with creating an action plan.
12. Create Your Action Plan, remembering to make goals that are measurable and realistic.
13. When you have finished creating your action plan, you should see a screen that states
you have finished the checklist quiz. If you are able to print at this time, click Next. (if not
able to print, exit the screen and come back to it when you are able to print).
14. Print this screen which includes quiz responses and action plan (or save as PDF). Submit
this to CCR&R either by mail or email.
15. Implement your action plan and make improvements! You may retake the Checklist Quiz
once you have completed your first action plan to see what other areas you may need to
work on, or if you have met best practice!
To Receive Let’s Move! Child Care
Recognition Award
1. If you have completed all steps in your action plan, met your goals, and believe you should
be eligible to receive $150 incentive, go to healthykidshealthyfuture.org and click on
“Take Our Healthy Checklist Quiz.” Sign in with email address, and retake the quiz.
2. If you have met all of the Let’s Move! Child Care goals, you should see a screen that says
“We are excited to award you the Let’s Move! Child Care Recognition Award,” after quiz
completion. Immediately below this statement is a link to download the Recognition
Award in a pdf document.
3. Download this award and print (or save). Submit to CCR&R by either mail or email.
4. If you did not receive this screen but instead are prompted to create another action plan,
you may continue to work towards the 5 goals until best practice is achieved. Remember,
you will still receive a $100 incentive by submitting and working toward at least one
action plan.
