Welcome to A New Leaf’s Triangle Play. We are delighted to share this fantastic period of discovery with you and your
child in the spirit of community. This class is designed for children 1 to 3 years old. Older and younger siblings may come
and participate at the same time. The tuition for the class covers all siblings with parents or caregivers.

Tuition of $75 for 3 Fridays of your choice, $150 for 6 Fridays of your choice, or $200 for all 9
Fridays.
You may send this contract and check ahead of time, or bring them on your first class.
Some classes will be at A New leaf, and some classes will be field trips. Call or text 615-5859790 if you are unsure of our location for a particular week.

When you sign this contract, you are confirming the following:
I agree to participate in the Triangle Play program with my child under the guidance of A new Leaf’s teachers. I
understand that if my behavior (or my children’s) or language during the class is inappropriate, I may be asked to
terminate my participation without refunds. I agree with A new Leaf’s policies, terms, conditions and requirements, and I
will abide by them. I have provided all information required, and paid the tuition. I agree to pay tuition as stated in this
contract. I understand that I will not receive a refund for tuition paid even if my child does not attend, withdraw, or must
leave the program. I understand that payment is due on the first class (or before) and will not be adjusted even if my child
misses days because of sickness, vacation, or other reasons, or when A new Leaf closes because of weather conditions.
I understand that make-up days will not be offered. I grant permission to Elle (Hélène) Harvey and other A New Leaf
employees to care for my child and provide first aid. I understand that I am (or my caregiver is) responsible for the safety
of my children while they participate in the Triangle Play program. Risk may include falls from the playground or logs,
animal encounters, chocking, stings or bites, falling tree limbs, heat, frost, and more. I understand that this list is
incomplete and that risks may result in injury, illness, or death. I hereby agree, acknowledge and appreciate that I release
and hold harmless with respect to any and all injury, disability, death or loss or damage to person or property, the
following named persons or entities, herein referred to as Releasees: Helene Harvey, A New Leaf’s directors, employees,
and volunteers. This release shall be binding to the fullest extent permitted by law. I have given up legal rights by signing
it, and I sign it freely and voluntarily without any inducement. I, as Parent, Guardian, Temporary Guardian with legal
responsibility for this participant, do consent and agree not only to his/her release of all Releasees, but also to release and
indemnify the Releasees from any and all liabilities incident to his/her involvement in these programs for myself, my heirs,
assigns, and next of kin. I further agree to indemnify, hold harmless and defend the Released Parties from and against
any loss, damage, liability, expense, costs, and/or attorneys’ fees. I also grant permission for my child to receive
emergency medical treatment by a paramedic, pediatrician, or emergency room physician, and I consent to all necessary
appropriate medical treatments.
