Mission Statement
To enhance the knowledge, skills and game play of all individuals and teams who
love the game of basketball, regardless of financial status, basketball background
and limited availability to play. We at Centercourt offer a unique program that fits
everyone's desire to play!

Open gym is designed for players of all skill levels to come, relax and just play fun basketball. Work
on your shooting, dribbling, pass with a partner or play in a pick-up game. A Centercourt rep will be
there at all times to ensure a safe & fun environment.

Centercourt Season Fees
$99/Mo for length of season (usually 3-4 months) Our seasons will include the following:






1 One on One Training (basketball- skille based)
Tournament costs split equally among players (ex. $200 tournament fee, 10 players=
$20/per player). We generally play in two tournaments a month. Travel tournaments and
costs will be separate from season fees.
$16 AAU Membership (required and is for the remainder of current year).
$20 Centercourt Membership (not required, but recommended).
$50 Centercourt Uniform (New uniforms may be required yearly or every other year). This
also includes last name on back of jersey. Some seasons jerseys are provided!
*Centercourt will NEVER turn away a player for financial burdens. Please see our Player Sponsorship Program (PSP) for more details.

We are currently seeking financial sponsorship to help with uniforms, leagues, tournaments, gym fees, travel, equipment and other
expenses. I hope Centercourt Basketball Organization can count on your support. Centercourt Basketball Organization has various levels
of sponsorship that any person, business, or company can participate in (sponsorship packages available upon request).
Dear Prospective Sponsor:
The Centercourt Basketball Organization was created with a vision for boy and girl players who desired to learn and play the game of basketball
on a very competitive AAU and club level. The goals are to provide a well-rounded learning atmosphere both on the hardwood and classroom. In
addition to the dedicated training, we as coaches and administrators strive to create and implement an environment that continues to promote
healthy living, individual confidence, self-esteem and leadership skills.
Centercourt Basketball Organization is looking forward to the 2017 spring, summer and fall seasons, which will consist of approximately 4-8 local
tournaments per season, as well as, offer our players and families several opportunities to travel and compete in top-rate tournaments around the
state and the entire northwest region.
As we continue to provide an environment of learning proper skills to become effective basketball players and life skills to grow responsible
adults, we do incur expenses to include tournament fees, uniforms, hotels and transportation and several others. The average expense per player is
approximately $500.00. While most players can afford the cost of the club level experience, some cannot. While we do have regular fundraisers to
help offset the costs for families in need, we still must seek community and local businesses for additional support. With your donation, your
organizations will get a tax deduction and advertisement on our website and publications. Centercourt Basketball Organization players are
counting on your company to help keep our team on the court. Be assured that your generosity will pay off with increased business when game
attendees see your company name featured on all Center Court gear.

Terms of Contract
I
would like my child to play the season circled above with Centercourt. I understand that this is a contract between
myself and Centercourt and promise to pay the total amount of $99 a month plus tournament fees (split equally). In exchange for the cost to
play, my son/daughter will receive the following.
-12 practices (1-2 hour per practice)
-12 classroom/film sessions (1 hour per session)
-12 Strength and conditioning workouts (1 hour per session)
-1 one on one training session (basketball, skill based)
-6-8 local tournaments
-Insurance coverage (practice, games) *Must Have Valid AAU Card
Payment Option
I agree to pay the full amount for the season whether it is $297 or $396 (or pro-rated amount). This is usually paid at a rate of $99/mo. Payment
for the month is due five days after receiving invoice, usually but not limited to the 1st-5th of the month. If not paid within five days, another
notice reminder will be sent. If payment is still not rendered, your child will be suspended from the team until further notice of collections.
AAU Form
Centercourt is an official AAU Club Affiliate. With that, we get great insurance and marketability. Therefore, each player and coach must have a
current AAU card. This can be found at: http://www.aauboysbasketball.org. Our club code is: WWF8T6. Players MUST turn in their receipt along
with all other player forms.
I have fully read and understand the terms of this agreement. I understand that refunds will not be given once season has begun. If for any reason Centercourt is not able to aquire enough
players to form a full team (8 or more) before the first practice date, a full refund will be given and the season will be cancelled. By signing below I agree that I have fully read, understand, and
agree to the terms of this agreement.

General Liability Release of Claims
Notice to Parent or Guardian: Do not sign this agreement before you read it. I acknowledge that my child will not be permitted to
participate in Centercourt Basketball Organization if I do not agree to the terms of this waiver.
I, _________________, would like my child to participate in Centercourt Basketball Organization. I understand that basketball
involves physical contact between players, that serious accidents or even death can occur during such sporting activities, and that
participating in such sporting activities can result in serious personal injuries, death, and/or property damage, as a consequence
thereof, knowing the risks of participation, nevertheless, I hereby agree to assume those risks on behalf of my son/daughter and
release, hold harmless and forever discharge Centercourt Basketball Organization, their agents, employees, successors assigns, and
their respective heirs, personal representatives, affiliates, successors, sponsors, and any and all persons, firms or corporations liable
or who might be claimed to be liable, whether or not herein named, from any and all claims, demands, damages, actions, causes of
action or suits of any kind or nature whatsoever, whether known or unknown, fixed or contingent, which I now have or may
hereafter have or claim to have, as a result of or in any way relating to the following: Centercourt Basketball practices, tournaments,
events, and any other Centercourt function.
I have carefully read this waiver and release and fully understand that it is a release of liability. I am signing this on my own free
will. I am also aware and acknowledge that I am waiving any and all rights that I may have to bring a lawsuit in which I could assert a
claim on behalf of my child against Centercourt Basketball Organization and all the other persons mentioned above for any injuries
or damages caused by the negligence of the aforementioned parties.
This Release has been read and fully understood by the undersigned and has been explained to me.
