The SMU Excellence in Student Life Award (President’s Award, Gold, Silver & Bronze) honours individuals in
recognition for their excellence in terms of the sum of their total contribution for the year to student life across Arts, Special
Interest Groups, Sports and/or Student Leadership.
Period of Assessment
The period of assessment for the Awards is: 1 Jan – 31 Dec 2016. Contributions made outside of this period will not be
considered for this year’s awards nomination and selection.
Instructions:
 Please use the respective forms (Category: Arts, Special Interest Groups, Sports, Student Leadership) for your
application / nomination. You may submit more than one application form if you have contributions /
achievements in the different categories.
 Please submit any relevant documents to support your application
