Preparing for submission of a League Safety Plan to District 33 and Little League
Two Copies of your league’s complete Safety Plan to the January District meetingone for Little League Intl. and one for District 33. You have the option of submitting
one printed copy for LL Intl. and emailing the District 33 copy of your entire Safety

If you email it early to Steve Taylor you can have it reviewed prior to printing a
hard copy.

Identify in this form the page numbers in your League Safety Plan where
each requirement can be found. This form can found and filled out online at
littleleague.org, then hover over League Officials button, select Forms &
Publications, then ASAP to get to this form (and other pertinent forms).

Note that Excel form has tabs on the bottom for each of the 5 pages.
Each Excel page must be printed separately (when it is open). The excel
version can be saved on your computer and used for reference the following
year.
* The insurance forms and posters can be accessed via littleleague.org (hover
over League Officials), click on “Forms & Publications” then ASAP. The point of
including the volunteer and insurance forms and posters in the two plans you
submit is to prove that you know of them and are presumably accessing them.
Beyond that, it is expected that sets of insurance forms will be available to
Managers as part of their Safety Plan as well as at the concession stand.

I have added two signature pages for league retention after the Cover
Page of the Model Safety Plan. Copies of the same pages are embedded into
the coaches portion of the Safety Plan: 1) Acknowledgement of Manager
Responsibilities, 2) Coaches Code of Ethics. Both pages require signatures on
the league copies (to be kept on file by the league through the end of fall ball).
At Clay Berry’s request I have added a section on AEDs in the Model Safety
Plan, page 12. If you have and AED, read this closely to make sure it is
consistent with your maintenance practices. If you do not have an AED, delete
this section from your plan but make sure Handling Emergencies stays on
page 13 so the Table of Contents is consistent.
Your Safety Plan will not be passed by Little League until initial player roster
data has been submitted for this year. Each time you submit revised data it
wipes out the previous submission, so make each submission a complete set.
FYI: Revise data to Little League if players are added or drop out after you sent
in initial info. Same for managers, coaches or league officials. This ensures they
are covered by Little League insurance through the end of December.
Send a revised complete data set, as all previous data is wiped out when new
data is submitted.
The Safety Plan posted on the District 33 website is a model for a league safety plan and includes
all sections listed below:
Cover
Body of the Safety Plan: includes Coaches Code of Ethics, District 33 Parent Code of
Conduct, Volunteer form, Coach training dates, Insurance Forms, Injury Tracking and
supporting Posters/Signs relevant to coaching.
Concession Stand safety information and related Posters/Signs.
The posters/signs that are pertinent to your game sites, equipment sheds and concession stands
should be printed and put into plastic sleeves and posted for reference in the appropriate places
around your league to increase safety awareness.
If you are starting with the Model Safety Plan, please download this year’s
version from the CA District 33 website “Forms” button, as I have made
numerous revisions and re-numbered the Table of Contents.
If you use this model as a safety plan for your league, you must read it entirely/modify it to make
sure it is consistent with the practices in your league, revising as necessary.
Pay attention to the Concession Safety practices. Use “Find and Replace” to save yourself
work if you are starting with the Model Safety Plan. You can get rid of the highlighting
once you have paid attention to all of those areas.
You can use your League’s own, previously developed, Safety Plan as long as it is updated to
include all of the required items (as per Safety Plan Registration 2017) and includes examples of
the insurance forms and posters that are appropriate. Remember to update Safety and
Fundamental Clinic dates for 2017. Revised language for Volunteer background checks in the
2017 model plan!
Printed copies to District 33 and Little League (3-Part District copy can be emailed):
1) The Safety Plan Registration Form 2017- 4 pages completed. It identifies many of
these Insurance Forms/Posters and Volunteer Form as required parts of the overall plan,
thus they should be included in your League Safety Plan submitted to Little League and
the District.
2) Your full league Safety Plan starting with the Cover page and Supporting docsInsurance forms, Volunteer form, Posters… from Little League Online
3) 2017 Facility Survey- all five pages completed
If you wish to pre-check your printed copy before submission to LL and CA D-33, on the
CA D-33 website in “Handouts” is a Check-off sheet I use before submitting plans to Little
League. You may do your own self-check by running through this list
Remember to complete the five-page Facility Survey for 2017. You help yourself if you
download the Excel version of this, as you can type in your information and save it for reference
next year. You have the option of submitting it online, but if you do it in Excel and save it, you
help yourself.

LL ONLINE INSURANCE FORMS AND POSTERS
For the several Insurance Forms, 2017 Volunteer Forms, Safety Posters… go to Little League
Online/Forms & Publications/ASAP and scroll down. www.littleleague.org (Little League
Online)
When printing forms and Posters from Little League Online, make sure you select 100%
when you print, as they seem to come up at 85%.
OTHER RESPONSIBILITIES
Promote safety in the actions of others- think safety at all timesMake sure Safety Plans are with each team (often in equipment bag) and all Medical Releases for
a team are completed. Many leagues provide zippered notebooks for teams that play interleague.
No horseplay during practice or at the game site (young people get carried away, go too far)
Bats and baseballs stored safely when players are in the dugout
Promote safe food handling procedures to your Snack Bar volunteers- one instance of illness
perceived by your fans to be due to tainted food from your snack bar will kill sales for the
season.
