PROGRAM DESCRIPTION
TARGET AUDIENCE:
Hospitals across the U.S. continue to grapple with
overcrowded emergency departments (EDs) and rising
outpatient visits and admissions. Building new facilities
takes time and money…and in the meantime, patient
care can suffer. For the past two-and-a-half years,
Lehigh Valley Hospital and Health Network (LVHHN)
has been sharing some of its strategies for managing and
maximizing capacity. In response to a growing demand
from other hospitals around the country for more indepth information, LVHHN is pleased to offer a twoday seminar on the topic. Participants will learn:

learning and innovation project managers, business process design, nursing management, emergency department,
management engineering, patient financial services.

In this session, you will learn about Patient
Logistics. Formerly known as “bed management,” patient logistics is a re-engineered
process that centralizes those departments
that affect patient flow including: inpatient
admitting and registration, patient transport,
housekeeping (related to discharge and
transfer beds), ambulance transport, acute
external hospital transfers and system administration for capacity management software.

Lehigh Valley Hospital’s multi-year, organization-wide commitment to maintaining an
open access health care system, was designed
to increase physical capacity and improve
patient flow processes while cultivating
leadership capabilities. The primary goal,
creating capacity to meet forecasted admissions growth, was realized and surpassed.
This presentation will provide a contextual
overview, from problem identification and
creation of an organizational infrastructure
to key project implementation including
measured results and organizational learning.

This session covers the concept of centralized bed control. Placement strategies and
prioritizing admissions by source and location will be highlighted. Methods for rapid
assessment and communication of hospitalwide patient flow will be presented including:
bed alert system, projected census with
electronic bedboardTM, mobile communication of key patient placement elements
(pending and confirmed discharge, isolation
needs and other special needs, patient
attributes).
10:15 a.m. Pull Through the ED and Beyond
Richard Mackenzie, M.D. – Vice Chairman,
Emergency Services
Dr. Mackenzie will share a fresh, new way of
looking at and improving patient flow in the
ED and through the hospital, including a
simple way of applying queuing theory to the
ED. He will focus on the effect that capacity
management software has on communication
of inpatient bed status and ultimately hospital
flow. Lehigh Valley Hospital’s success in
reducing the average ED length of stay by
60 minutes while experiencing an increased
network-wide admission volume of 8% will
illustrate the impact of pull systems and
technology-accelerated communication on
the care of patients in the ED.

PACU Alert
POD Pull Project (P3) — that have
accelerated patient flow through the ED
and reduced ED lengths of stay. Strategies
included pre-assigning incoming ambulances
to ED bays; using an electronic ED tracking
system to facilitate flow; “pulling” patients
from the waiting room to the main; and
assigning a second triage nurse to the
waiting room.

This session presents PACU Alert, a datadriven approach to addressing issues with
patient flow from the Operating Room (OR)
to the recovery room (PACU). Responding
to increasing OR hold time, PACU Alert
employed a multidisciplinary team that
investigated root cause, then designed and
implemented a strategy to reduce hold
times by 75%. One year after implementation of the PACU Alert system, an
85% reduction in OR hold times has been
achieved.

This session focuses on direct admission to
acute care beds. It will highlight strategies
for streamlining admissions from physician
offices directly to an inpatient bed, allowing
clinically appropriate patients to bypass the
ED. Presenters also will cover creation of an
Express Admissions Unit for direct admits as
well as those patients admitted through the
ED awaiting inpatient beds.
ED Alert

This session will cover the ED Alert, which
employs an alpha-page system to mass communicate with 100+ individuals instrumental
to patient flow. Using a four-tiered approach,
certain triggers — ED waiting room time,
the number of patients awaiting beds and
acuity — initiate the alert. This proactive
approach anticipates demand and enlists help
from a multidisciplinary team extending
beyond the ED to maintain flow. The team
comprises patient care unit directors, flow
coordinators, patient logistics’ patient placement staff, patient transport and housekeeping.

The Lehigh Valley Hospital Transfer Center
coordinates 400+ acute hospital referrals per
month. Presenters will discuss the transfer
center model plus protocols for transferring
patients with acute myocardial infarction,
stroke and neurosurgical emergencies. They
also will cover the use of data to manage
referral patterns and track lost business.
