The deadline for completed and signed entries is 14.02.13. Please ensure your form has been emailed to
[email protected] prior to close of play on 14.02.13

Please fill one form for each different card product submitted. So use one form to submit the same product in more than one
category but different forms for different products. A separate form should be used for Business category submissions.

Entry Fee: Select number of consumer or other categories or business categories entered and invoice will be generated upon
receipt of submissions.
Example Scenarios

Submitting the Orange Pay As You Go Card in 3 Categories qualifies as one submission and would be charged £90 but the
Orange Pay Monthly Card must be submitted as a separate entry.

The Bread card can be submitted into any number of categories as one submission but the Bread Euro Travel Card needs to be
submitted as a separate entry on a separate form.

The PFS Prepaid Card can be submitted as a consumer card in the consumer and other categories but the company PFS should
be submitted on a separate form for Business Category Submissions
