Vedlegg Department of Bioscience FORM The Faculty of Mathematics and Natural Sciences Skjema sendes studieadministrasjonen Med kopi til HMS-k og HMS-i Risk assesment of MSc-thesis The supervisor is responsible to submit a short risk assessment together with Masterkontrakten this is achieved at the student administration. UiO has many common procedures and tools to ensure a safe working environment this is the basis of the systematic HSE at UiO1. In addition Department of Biosciences (IBV) developed local procedure to clarify and elaborate clarify2, this are available at the IBV HSE webpages. For riskassessment and reporting deviations (e.g. development of allergies or exposure to a GHS08 chemical) refer to: Risk assessment procedure 3, Procedure for writing a Standard Operating Procedure with a risk assessment (SOP)4 og Procedure for management of adverse events 5 What should be risk assessed? When working with chemicals, hot work, biological agents, genetically modified microorganisms, factors harmful to reproduction, noise, mechanical vibration and/or radiation sources a risk assessment should be performed. Substition To replace dangerous chemicals and processes that is one of the most important tasks to protect the workers and stydents against risk associated with work with chemical and biological agents. For carcinogens, mutagens, reproductive chemicals the regulations to replace this chemicals is very is very cancergenic and mutogenic chemicals For kreftfremkallende og arvestoffskadelige stoffer er kravene om erstatning spesielt strenge. Bestemmelsene om erstatning er fastlagt i forskrift om vern mot eksponering for kjemikalier på arbeidsplassen (Kjemikalieforskriften). Dette er tatt med i SOPene av nyere dato. Where do you find the risk assessments? At IBV all scientific and educational use of chemicals, biological agents (bacteria, virus, fungi etc), physical factors and radiation sources are described in our SOPs. In the SOP you'll find a written procedure with risk assessment and waste disposal routines. All SOP's are written in English. A search tool is also available; you can find all SOPs that contain certain words/CAS number, chemical name, bacteria strain etc. The SOP's are divided into: P-SOP: Procedures that describe a general method for e.g. DNA-isolation, use of various solution etc. S-SOP: Procedures that describe how to make various solution e.g. 1M HCl, 6% perchloric acid, 1M TRIS etc. This also includes new CLP-labelling of the produced solutions and dilutions. 1 Policyer, prosedyrer og verktøy som trygger din arbeids-plass ved UiO er beskrevet her: http://www.uio.no/om/hms/ HSE at IBV: http://www.mn.uio.no/ibv/english/about/hss/ 3 Risk assessment: http://www.uio.no/english/about/hse/working-environment/procedures/risk-assessment/index.html 4 SOP: https://www.mn.uio.no/ibv/english/about/hss/prosedyrer/procedure-sop-riskassesment-1.html 5 Deviation/Adverse events: http://www.uio.no/english/about/hse/working-environment/procedures/adverse-events/index.html 2 1 vedlegg Institutt for biovitenskap Skjema Det matematisk-naturvitenskaplige fakultet Date: Name supervisor: Department/section: Name student: Tittle MSc thesis : Please list the most relevant procedures the student must familiarize themselves with. The procedures are available on IBVs HSE web-page. Adverse events Biological substances (Safety precausions to pretect against infections, allergies and toxicity) Chemical inventory Disposal of chemicals Genetically modified organisms (GMO) Hazardous waste HSE training HSE training and access control KB house Purchase, use, storage and transport of chemicals Risk assessment Safety equipment for work with chemicals (fume hood/point suction) Safety equipment for work with biologiske substances (safety cabinets) Waste disposal Writing a SOP with risk assesment (SOP) Is there special risk involved in this MSc work? This MSc the student will work with (remove the boxes to make the list relevant for the MSc work): ☐ Radioactive sources (Exposure to radioactive substances) ☐ kjemikalier merket med GHS08 (Exposure to chemical substances) ☐ klasse II bakterier (Exposure to biological hazardsous class II) ☐ animals (Exposure to allergens (mice/rats) and risk of development allergies/asthma) ☐ GMO plants (Exposure to allergens (plant pathogen-mould) and risk of development allergies/asthma) ☐ Field work 2 Opplæring: All students - both student with a bachelor from UiO and other institution - must take the mandatory HSE courses. HMS0503 – Laboratory safety (mandatory for BIO/MBV) HMS0504 – Field safety (mandatory for BIO) For some master thesis other HSE course may be relevant (remove the boxes to make the list relevant for the MSc work): ☐ HMS0505 - El-safety ☐ HMS0506 – Patient clinic safety ☐ HMS0507 – Fire safety List risk assessed procedure relevant for this MSc.: For eksempel: Quantification of DNA (dot-spot) P-SOP-102 Multiphoton imaging of wildtype and GMO rodents P-SOP-103 Secure use of GMM in combination with rodents P-SOP-104 Rapid DNA extraction using REDExtract-N-Amp Plant PCR Kits (Sigma) P-SOP-105 List procedure relevant for this MSc., that need to be risk assessed: Summary of the risk assessment: Risk of development asthmas and allergies (If this is a relevant problem for this master thesis specify here) Risk of exposure to biological hazards (If this is a relevant problem for this master thesis specify here) Risk field work hazards (If this is a relevant problem for this master thesis specify here) Risk of exposure to biological hazards It is assumed that the student follows the precautions described in SOP and MSDSs. All chemical are labelled according to CLP. There will always be a risk of exposure to the chemicals that are used and stored at the location, even if the students and other staff and students followed the necessary precautions. If the student is exposed to chemical the student will contact the supervisor or other staff to ensure that the incident are reported according to procedure. For chemical that are labelled with GHS08 this must also be reported to NAV (and SKP if relevant). 3 Best regard, ___________________________________ Supervisor 4
