I am sending you an Invitation For Bid and Product Bid Sheet for the purchase of a VoIP Phone System
for the Hart County Health Dept. located at 64 Reynolds Street, Hartwell, GA 30643. Please see our
website at www.phdistrict2.org for more details. If you are interested in bidding and do not have the
exact specifications, please bid a comparable product (see website for details). Please submit bid price and
return the completed Product Bid Sheet in a sealed envelope with “SEALED BID” printed on the outside
of the envelope. If you do not wish to bid, please complete the bid sheet and print “NO BID” in the space
provided at the bottom of the page and return in a sealed envelope with “SEALED BID” printed on the
outside of the envelope. Submit your completed RFP in a separate envelope, see RFP for details.
Bids will be opened at the Hart County Health Dept. located at 64 Reynolds Street, Hartwell, GA 30643, on
April 5, 2012 at 10:00 am. You may be present for the bid openings. Your signature is required to make this
bid legal.
