FORM #1 NMP Summary Report Instructions Refer to the NMP Worksheet you have completed and enter figures into the NMP Summary Report (Form #3 & #4) Online video tutorials are provided on the Coalition’s website at https://www.esjcoalition.org/nmpTraining.asp 1. On Form #3 fill in the Management Unit names for the parcel(s) you are reporting. This can be the actual Assessor Parcel Number(s) or management unit(s). NOTE: Include information from each certified NMP Worksheet you completed on a separate line of the NMP Summary Report (Form #4). Each NMP Worksheet is based on similar crop and nitrogen use rates. 2. Record the Specific Crop from Form 5 for each parcel in the “Specific Crop” column on Form 3. The Specific Crop List is important to review for farmers with perennial crops. We need to know the age of the crop and the purpose of the crop as it results in substantial differences in yield. 3. Enter the irrigated acreage for each parcel from box #10 on Form #3. 4. On Form #4 fill in the Crop Year from box #1 and your Member ID from box #2 of the NMP Worksheet (This information is for Coalition tracking purposes ONLY.) 5. Enter your name and the date that you are filling out your NMP Summary Report in the space for Submittal Date and Forms Completed By. 6. Record each management unit name from Form #3 in the Management Unit column on Form #4. 7. Sum the total irrigated acreage associated with each Management Unit and record it in the Total Acres column. 8. Record the Specific Crop associated with each Management Unit. 9. On Form #4 in the Total Available N Applied column, add the ACTUAL Total Available N Applied and the N in Irrigation Water values together (shown in blue below). For A/Y, divide the Total Available N Applied per acre (calculated above) by the Actual Yield per acre ([box #22 + box #25] ÷ box #11). All calculations are on a per acre basis. 10. Fill in the production unit (box #7) for the yield Production Unit. Pounds or tons are the preferred production unit. If you report in units other than pounds or tons please use the Production Units Cheat Sheet to convert your yield to pounds. If you returned an NMP Summary Report for 2015, the Management Unit, Specific Crop type and the acreage will be prepopulated for you. Re-group Management Units, APNs, or parcels as needed. “Management Unit” is the description or name given to the property listed on the sheet as a group. You can also use the Assessor Parcel Number (APN) as a descriptor of the Site Location or Management Unit. (See Form #2 example sheet.) If you do not apply nitrogen fertilizer to your fields these forms are still required to be returned. Simply fill in the crop type, acreage, and on Form #4 put zero for nitrogen applied. If you are willing to provide the yield per acre that would be helpful to show how much nitrogen you are removing from the soil. If APNs are leased by other farmers, it is the sole responsibility of the member to make sure this information is provided. Please contact Coalition Membership Services (209) 846-6112 to update contact information for parcels leased to other farmers. Updating this information ensures that the Coalition receives all the necessary information for all High Vulnerability parcels. The concentration of nitrate can then be extrapolated to determine the pounds of nitrogen per acre foot of water. How to convert between NO3 and NO3-N: To convert Nitrate (NO3) to Nitrate-Nitrogen (NO3-N), multiply by 0.2259. To convert Nitrate-nitrogen (NO3-N) to Nitrate (NO3), multiply by 4.4268.
