Recommendations to Consider when
Submitting Documentation
 The Management Review Checklist (1011) should be supported by minutes of the
Management Review Meeting (i.e.,
minutes should cover topics discussed in
the meeting including any legal changes
and developments).
 The Continuous Improvement Log (12013) should support the continuous
improvement program (i.e., projects
should provide evidence that an agency is
working on problems to reduce Nonconformance Trends (102-1 & 102-2).

Page 353
Recommendations to Consider when
Submitting Documentation
 When the Internal Audit Checklist (15031) indicates that a category “Needs
Improvement” or is “Unsatisfactory,” the
issue should show up on the appropriate
logs (1201-2 & 1201-3). This provides
evidence that the quality management
system is working.
 When a problem shows up on the Client
Issues Log (302-2) or the Non-conformity
& Corrective Action Log (1201-2), it would
be expected that the submitted Nonconformity & Corrective Action Report
(1201-1) would be traceable back to one
of these logs.

A. Description of Nonconformance:
Severity: ___Major (needs to answered in 90 days)
___Minor (needs to answered by next submission)
Nonconformance - an observation that indicates policy or practice contrary to the evaluation criteria.
 Major Nonconformance
- A long-standing minor nonconformance from previous evaluations
- Established as detrimental to the service delivered to the client

Complete and return the application to ACA International.
ACA will then send you a copy of the evaluation agreement. Sign and return this agreement with the appropriate fees.
Once the application, evaluation agreement and fees are received, you will be asked to send the required documentation directly to
CliftonLarsonAllen, LLP. Please don’t send any documentation to CliftonLarsonAllen, LLP until you have received notification from
ACA that your application process is complete.
To achieve certification, various record logs need to be attested to by an independent CPA. Attested means the records that support
various logs are on file. It does not mean an evaluation was made to express an opinion. An independent CPA means a Certified
Public Accountant that is not involved with or has influence over the agency’s business (i.e., the CPA cannot be an employee of the
agency). The independent CPA shall sign and submit a letter/report supplied by ACA stating their independence and that they
followed the prescribed procedures required by the ACA PPMS™ Committee. Agencies shall select and compensate the CPA of their
choice for services rendered.
* If you are applying for multi-site certification, please contact ACA International for additional requirements.

Recommendations to Consider when Submitting Documentation:




The Management Review Checklist (101-1) should be supported by minutes of the Management Review Meeting (i.e., minutes should
cover topics discussed in the meeting including any legal changes and developments).
The Continuous Improvement Log (1201-3) should support the continuous improvement program (i.e., projects should provide
evidence that an agency is working on problems to reduce Non-conformance Trends (102-1 & 102-2).
When the Internal Audit Checklist (1503-1) indicates that a category “Needs Improvement” or is “Unsatisfactory”, the issue should
show up on the appropriate logs (1201-2 & 1201-3). This provides evidence that the quality management system is working.
When a problem shows up on the Client Issues Log (302-2) or the Non-conformity & Corrective Action Log (1201-2), it would be
expected that the submitted Non-conformity & Corrective Action Report (1201-1) would be traceable back to one of these logs.
