About this Document While online, PDF-based forms are common place and generally familiar to most internet uses, the Referees’ Game Report E-Form includes functional capabilities that make completing, saving, printing, and submitting the your reports much simpler than in the past. This document provides: Prerequisite tools and technologies required to complete and submit the form. Background information to give you a basic understanding of how the form is processed. Procedures for preparing to complete, completing, and then submitting your form. 1.2. Prerequisites The following tools and technologies are required prerequisites for accessing, completing, and submitting the Referees’ Game Report E-Form: Internet access. Web browser Adobe Reader or Adobe Acrobat A Digital Signature/ID Adobe Reader is available as a free download (click here for Reader) while Adobe Acrobat must be purchased (click her for Acrobat). Adobe Reader is sufficient for accessing, completing, and submitting the form. A Digital Signature/ID can be created using either Adobe Reader or Acrobat. 1.3. How it Works The new Referees’ Game Report E-Form and the associated submission process is intended to streamline and simplify the handing of game reports while getting the form into the hands of both the York USA League Administrator and the YASRA Referee Assignor as quickly as possible. As illustrated in Figure 1, the new process includes three basic processes, each of which consists of a series of manual and automated steps. 1. Create Your Digital ID and Signature (new users only) a. Determine how you want your Digital Signature to appear when electronically signing your Referees’ Game Reports. b. Create Your Digital ID “self-certifying” your ID serving as proof of your identity. c. Customize the appearance of your Digital ID as it will be used as your Digital Signature. d. Save your Digital ID and Signature for use when signing your electronically submitted Referees’ Game Reports (your Digital ID is actually saved after it is created and then again saved automatically when you apply an appearance creating your Digital Signature). 2. Go to the York USA website (www.yorkusa.org) and open the Referees Game Report E-Form (Game Reports Referees’ Game Report). b. Enter your report data in the fields provided on this electronic version of the Referees’ Game Report form. c. Digitally sign your game report using the Digital ID and Signature created in the Create Your Digital ID and Signature process. d. Click the “Submit Report” button on the bottom of Page 2 of the completed electronic game report. 3. Submission Process (a series of manual and automated functions). a. The process first automatically launches the Adobe Save dialog prompting you to save a copy of your completed game report to your computer. b. Next, the process automatically launches the Adobe Print dialog prompting you to print one or more copies of your completed game report for submission to the League Office along with your Game Line-up Sheets using the USPS. c. Finally, the process automatically launches the Adobe Email Client selection dialog prompting you to email a copy of your completed game report to the YASRA Referee Assignor. About Digital IDs and Signatures 1.4.1. Digital IDs A Digital ID is like a driver’s license or passport. It proves your identity to those people and organizations that you communicate with electronically. A Digital ID usually contains your name and email address, the name of the company that issued your Digital ID, a serial number, and an expiration date, or similar forms of data. You must have a Digital ID to sign and submit the Referees’ Game Report E-Form electronically. NOTE A self-signed Digital ID and self-generated Digital Signature are adequate for signing the Referees’ Game Report E-Form. 1.4.2. Digital Signatures A Digital Signature, like a conventional handwritten signature, identifies the person signing a document. Unlike a handwritten signature, a digital signature is difficult to forge because it contains encrypted information that is unique to the signer and easily verified. Figure 2 illustrates a Digital Signature as it might appear on a PDF from where the shaded area represents to form's signature field. Figure 2 Digital Signature on a PDF Form Example Signatures that certify an Adobe® PDF are called certifying signatures. Only the first person to sign a PDF can add a certifying signature. A certifying signature attests to the contents of the document. Adding a Digital Signature to the Referees’ Game Report E-Form indicates that the submitting referee attests to the contents of the report and indicates that the report was completed and submitted by the referee whose Digital Signature has been attached to the form. To apply a Digital Signature to the Referees’ Game Report E-Form, you must obtain a Digital ID (fee-based) or create a self-signed (free) Digital ID. NOTE A self-signed Digital ID and self-generated Digital Signature are adequate for signing the Referees’ Game Report E-Form. Background A Digital ID and Signature consist of two components as shown in the two examples illustrated in Figure 3: Figure 3 Digital ID and Signature Examples 1. Signature: Identifies the signer and shown on the left-hand side of the Appearance, and may be in the form of a. Text: Where the signers name is shown in simple printed text. b. Graphics: Where an image (e.g., the signers written signature) is shown. 2. Identity: Data about the signer and shown on the left-hand side of the Appearance. 2.1.1. Approved Digital ID and Signature Constructs 2.1.1.1. Signature Component As illustrated in Figure 3, the signature component of your Digital Signature may be printed text of your name or a graphic representation of your written signature. 2.1.1.2. Identity Component The identity component of your Digital Signature must be formatted as indicated in Table 1. Select the location where you would like to store your new digital ID… The choice is yours but on Windows computers, the “Windows Certificate Store” is a good option. …then click Next. 9. Enter your Identity Data as follows… Name: Enter your first and last name Organizational Unit: Enter your current USSF ID# Organizational Name: Enter “York Area Soccer Referees’ Association” Email Address: Enter your email address Country/Region: Select US – UNITED STATES Enable Unicode Support Uncheck (do not select) the check box Key Algorithm: Select 1024-bit RSA Use digital ID for : Select Digital Signatures and Data Encryption …then Click Finish. 10. Close the Security Settings dialog. NOTE Do not close your Adobe Reader or Acrobat application, you will need the application for the next procedure. Customize Your Digital Signature Appearance Perform the following procedural steps to create your digital signature appearance using Adobe Acrobat. 1. Click Edit on the application menu bar then click Preferences from the dropdown menu. The Security Settings dialog displays. NOTE While the diagram shown is based on Adobe Acrobat, Reader Preferences are also accessed from the Edit menu. Determine the type of Digital Signature Appearance you want to create. If you want to create a… then… Text-based digital signature… Continue with Step 4 Graphic-based digital signature… Continue with Step 6 4. Enter a Title for your Digital Signature Appearance… e.g., “YASRA Referee Signature” …then click OK. 5. Continue with Step 10. 6. Select the Imported Graphic choice under Configure Graphic options. 7. Click the File button associated with the Import Graphics from: option in the Configure Graphic options to open the Select Graphic dialog. 8. Navigate to the Signature graphic you created in section 2.2.2 Create Your Digital ID and then click Select. 9. Click OK to close the Select Graphic dialog. The newly created Digital Signature Appearance is now displayed in the Preferences dialog. 11. Click OK close the Preferences dialog and save your newly customized Digital Signature Appearance. Introduction The Referees’ Game Report E-Form is two-page PDF file-based electronic form accessed from the York USA website. The form requires a Digital ID and Signature which must be applied to the report prior to submittal (refer to: Section 2 Create a Digital ID and Signature). The form is “submitted” electronically via email to the YASRA Referee Assignor and also submitted in printed paper format via the U.S. mail along with the associated game line-up sheets to the York USA League Office. Both the form’s data fields and report submission process include automated functions and features to make simplify the submittal process (refer to: subsection 1.3 How it Works).` 3.2. Access the Form Perform the following procedural steps to access the Referees’ Game Report E-Form. 1. Open your web browser. 2. Go to www.yorkusa.org. 3. On the left-hand, vertical menu mouse over Game Reports then click Referees’ Game Report from the pop-up menu. The Referees’ Game Report E-Form opens in your default PDF plug-in (Adobe Reader or Acrobat) within your web browser. The form should appear similar to the following with some differences depending if it has opened in the Adobe Reader or Acrobat plug-in and which version of the Adobe application is installed on your computer. Complete Your Report The Referees’ Game Report E-Form is two-page PDF file-based electronic form accessed from the York USA website NOTE If the form displayed in your browser does not include highlighted form fields as illustrated in the previous procedure, then, if you’re using: 1. Adobe Reader – a. From the Form Notification bar, click the HIGHLIGHT FIELDS button. 2. Adobe Acrobat – a. From the Acrobat menu bar, click FILE HIGHLIGHT FIELDS, or… b. From the Form Notification bar, click the HIGHLIGHT FIELDS button. 3.3.1. Complete Page 1 Perform the following procedural steps to complete page 1 of the Referees’ Game Report EForm. NOTE Some form fields require data to be entered in specific formats. For example: Score fields require numeric data. Date fields require date formatted data (e.g., mm/dd/yyyy). Dropdown menu fields accept only data from the dropdown menu. Signature fields require Digital ID and Signature data. Enter the club and team of the home team using the convention: [Club Code] [Team Name] E.g., Eastern York Raptors would be entered “EAS Raptors” The 3-character club codes can be found on the Member Clubs page of the York USA website. 3. Press Tab to move the cursor to the next field (Score). 4. Enter the number of goals scored by the Home team. 5. Press Tab to move the cursor to the next field. 6. Continue the process of entering data, pressing the Tab key until you have completed the Weather field. 7. The form includes 13 fields that are completed using drop-down menus. Beginning with the “Was the home team on the field on time?” field, select the appropriate response for each dropdown selection. Note that there are three text entry fields mixed in with the drop-down menu fields. 8. Enter a dollar value equal to any Referee Fee not received. NOTE In the Remarks field of page 2, add a brief note indicating: Which team(s) failed to provide a complete fee. Which referee(s), by name and role, (e.g., John Smith CR $41, Joan Smythe $5) 9. Click in the Referee Signature Field 10. Click in the Referee Signature field. The Sign Document dialog opens. Select the Digital ID to be used in signing page 1 of the form from the Sign As dropdown menu. 12. Select the Digital Signature appearance to be used in signing page 1 of the form from the Appearance dropdown menu. 13. Click the Sign button to apply the selected Digital ID and Signature to NOTE Applying an electronic signature to a document forces you to save a copy of the report before proceeding. 14. Save the Referees’ Game Report E-Form to a location of your choice (e.g., a “Referee Reports” folder) on your computer. TIP When saving a copy of your report, use a naming strategy that will help you and the Referee Assignor to easily identify the report contents. For example, [date(yymmdd)][division][gender][home team]V[visiting team] 120908U10FEASscorpionsVTROstars.pdf 15. Continue the process of entering data on page 1 3.3.2. Complete Page 2 Perform the following procedural steps to complete page 2 of the Referees’ Game Report EForm. NOTE Fields that are common to both page 1 and 2 (e.g., Teams, Scores, League, etc.) have autopopulated. 1. Scroll down to Page 2. 2. If necessary, enter any pertinent information in the Describe Any Unusual Incident field. NOTE The Referees Game Report E-Form does not support electronic illustration of any unusual incident on the form’s field layout diagram. If an illustration will help describe the incident, then add the drawing to the printed copy of the form to be sent to the York USA League Office and note here on the electronic form that an illustration was provided. 3. If necessary, add any additional information in the Remarks field. NOTE For security reasons, the electronic signature process does not auto-populate multiple signature fields within a single form. You will be required to sign, thereby certify the contents, of each page of your Referees’ Game Report E-Form. Select the Digital ID to be used in signing page 1 of the form from the Sign As dropdown menu. 6. Select the Digital Signature appearance to be used in signing page 1 of the form from the Appearance dropdown menu. 7. Click the Sign button to apply the selected Digital ID and Signature to NOTE Each time you apply an electronic signature to a document you must save a copy of the document with the newly added signature before proceeding. The Save dialog box opens and defaults to the file name and location that you used in Step 14. 8. Click Save to again save your Referees’ Game Report using the default location and file name provided. NOTE Do not close your Adobe Reader or Acrobat application; you will need the application for the next procedure. Submit Report Perform the following procedural steps to submit your Referees’ Game Report E-Form. 1. Click the Submit Report button on located on the bottom of page 2 of your completed and digitally signed form. The Save As dialog opens and defaults to the file name and location that you used when electronically signing your form. NOTE Any additions or changes made to the form data since last saving the form/adding your signature will not be included in the final copy if not saved again during the submission process. 2. Click Save to again save your Referees’ Game Report using the default location and file name provided. Your report is saved and the Print dialog automatically opens. 3. Make any print property changes needed then click OK to print a hard copy of your Referees’ Game Report. Your report is printed and Select Email Client dialog opens. 4. Select the type of email client you will be using to electronically submit your report to the Referee Assignor, and then click OK. Your selected email option opens as illustrated in Figure 5.
