Dear Applicant,
You are invited to apply to the Jack Lewis Invitational. The format is 2-Man Better
Ball. Entrants must be male, amateur contestants, 18 years of age or older.
TOURNAMENT SCHEDULE
1. Practice round on Friday. Contact Pro Shop for tee time. No tee times later than 4:30 p.m. The
practice round includes a "mini tournament" with prizes in excess of $750.00.
2. Tee times will be between 7:30 a.m. and 2:00 p.m., flighted after the first day.
3. The entry deadline is July 28, 2017 (postmark). Entries will be confirmed by August 4, 2017
at tyogacc.com.
4. A long drive contest will be held at 6:30 p.m.. ($250.00 prize money).
NEW SENIORS DIVISION
1. Same schedule and format. Contestants (2-man team) must be 60 years and older.
Participation in the Seniors Division is optional.
2. The Seniors Division will be flighted separately and teams will compete for their own trophy
and prizes. ENTRY FEE: $400 per team. Members with carts shall pay $380.00.

ENTRIES
1. Entrants shall apply on form attached. Applications may also be found at
www.tyogacc.com.
2. Every team shall pay a $420 entry fee (the entry fees covers cart fees for 3 days). Members
with carts shall pay $395.00. Additional tickets for guests are $25.00 per person. Dinner
tickets are required to be purchased in advance.
3. All participants must take a cart.
CONDITIONS
1. The Tournament Committee reserves the right to alter any of the conditions and schedules
herein. The decision of the Tournament Committee shall be final.
2. There can be no substitution of a team member after August 3rd. After that date alternate
teams will be selected based upon availability.
GENERAL INFORMATION
1. A back porch barbecue will be available for players on Friday evening, following your
practice round.
2. All club facilities are open to participants and their families on Friday, Saturday, and Sunday.
Please enjoy Tyoga's pool!
3. Saturday's dinner (7:00 p.m.) will immediately follow 6:00 p.m. cocktail hour and the long
drive contest.
4. All alcoholic beverages shall be purchased at Tyoga Country Club.
