APPLICATION FOOTBALL TOURNAMENT 5-A-SIDE
Complete this application to register your team for the FOOTBALL TOURNAMENT 5-A-SIDE in aid of
Fondazzjoni U, which is the organiser of the philanthropic TV programme 'Maratona Missjoni'.
The tournament is on Friday 26 August from 8pm onwards at the Mqabba Grounds in Triq ilMadonna tal-Ġilju. Each participation has a registration of €200 per team with the possibility of
winning €700. Medals and trophy shall be given to the winner and runners-up.
No more than ten (10) teams are accepted. Places are therefore limited. Fill in the underneath.

TERMS & CONDITIONS
 It is important that each player is responsible for himself. This is a fundraising activity in support of
the Missions. No compensation shall be given in case of accidents.
 Registrations with payment are received till 20 August. A receipt shall be given. Polls are drawn on 23
August at the Mqabba grounds (8pm). In case less than 10 teams register the prize of €700 can be
reduced.
 We will be having 2 groups of 5 each. The best two will move to the semi-final and then the final. In
case of same points at group stage, the goal average will count first and subsequently the direct
confrontation. Three penalties shall be given if the teams still remain tie. Each match is 20 mins long.
Final will be on 2 halves of 15 mins each.
 All referees are experienced. In case of bad behaviour a match can be suspended. Players given two
yellow cards are suspended for 3 minutes. In the case of a direct red card, players are expelled for
that whole match.
 Each team needs to be at the grounds at least half an hour before. If at least 4 players are not
present by match time a walk over of 3-0 will be given to the opponent, without compensation.
Compensation in case of cancellation shall be given only by the time the polls are drawn.
 Each team has a limit of 8 players.
 Players from premier and first division leagues are not allowed. Anyone caught registering such
players will be disqualified and no compensation shall be given.
