Season Pass 16/17
I understand, for myself and/or my child, and am aware that the sports of skiing, snowboarding, ski boarding and other similar sports (collectively
“RECREATIONAL SNOW SPORTS”) are HAZARDOUS activities that have many inherent dangers and risks. I realize that injuries are a common
and ordinary occurrence of these activities. I agree to for myself and/or my child, to RELEASE from liability, forever discharge, indemnify, defend,
and hold harmless Boston Mills Ski Resort, Inc., Brandywine Ski Resort, Inc., Sycamore Lake, Inc. DBA Alpine Valley Ski Center, Peak Resorts,
Inc. and their owners, officers, directors, volunteers, employees, affiliates, agents and assigns, (collectively as “RELEASEES”) from any and all loss
or damage to myself or to my child or my or my child’s property resulting from the RELEASEES’ NEGLIGENCE. I understand that the
RELEASEES are not responsible for the consequences of their own NEGLIGENCE, that is, their failure to use reasonable care. I acknowledge that I
am freely and expressly assuming and accepting any and all risks of property damage, personal injury and death as a condition of being allowed to
use the ski area facility and premises.
I understand that RECREATIONAL SNOW SPORTS involve inherent dangers and other risks of INJURY and DEATH. “Inherent dangers and risks
of RECREATIONAL SNOW SPORTS” means those dangers or conditions which are an integral part of the sports, including but not limited to snow
conditions as they exist or may change, collisions with natural objects or man-made structures and their components; variations in steepness or
terrain, whether natural or as a result of slope design, snowmaking or grooming operations, and collisions with other skiers. I hereby agree to freely
and expressly assume and accept any and all risks of injury while engaging in RECREATIONAL SNOW SPORTS at Boston Mills Ski Resort, Inc.,
Brandywine Ski Resort, Inc., Sycamore Lake, Inc. DBA Alpine Valley Ski Center, Peak Resorts, Inc. I further agree to defend and indemnify Boston
Mills Ski Resort, Inc., Brandywine Ski Resort, Inc., Sycamore Lake, Inc. DBA Alpine Valley Ski Center, Peak Resorts, Inc. for any loss or damage,
including claims or lawsuits for personal injury, loss of consortium, loss of services, death, and property loss and/or damage, relating to my, my
spouse’s or my child’s participation in RECREATIONAL SNOW SPORTS at Boston Mills Ski Resort, Inc., Brandywine Ski Resort, Inc., Sycamore
Lake, Inc. DBA Alpine Valley Ski Center, Peak Resorts, Inc. I release Boston Mills Ski Resort, Inc., Brandywine Ski Resort, Inc., Sycamore Lake,
Inc. DBA Alpine Valley Ski Center, Peak Resorts, Inc. from any and all responsibility or liability for injuries or damages while I am participating in
RECREATIONAL SNOW SPORTS at Boston Mills Ski Resort, Inc., Brandywine Ski Resort, Inc., Sycamore Lake, Inc. DBA Alpine Valley Ski
Center, Peak Resorts, Inc. and I agree not to make any claim against Boston Mills Ski Resort, Inc., Brandywine Ski Resort, Inc., Sycamore Lake, Inc.
DBA Alpine Valley Ski Center, Peak Resorts, Inc. for injuries or damages relating to RECREATIONAL SNOW SPORTS as noted by my signature
below.

Additional Pass Terms & Conditions
Initial:
_______ 1. I understand that there are NO REFUNDS.
_______ 2. I understand that this pass is non-transferable and is for the use of the designated passholder only. If I am caught giving my pass to
someone else to use, the pass will be voided for the remainder of the season and a processing fee will be charged. This is fraud.
_______ 3. I understand that the passholder (regardless of age) must be the one to present the passcard at each lift.
_______ 4. I understand that if I misplace or lose my pass, there is a $25 replacement fee. I may be responsible for the use of the pass between the
time when it is misplaced/lost, and the time that its absence is reported to Boston Mills Ski Resort, Brandywine Ski Resort or Alpine
Valley Ski Area.
_______ 5. I understand that safety is very important, and that this pass may be revoked by Boston Mills Ski Resort, Inc., Brandywine Ski Resort,
Inc., Sycamore Lake, Inc. DBA Alpine Valley Ski Center, Peak Resorts, Inc. for violation of safety or company rules.
