Our commissioning intention is to combine current funding (across various
contracts) from Adult Social Care and Public Health to jointly commission a service
to provide successful outcomes for both rough sleepers and street drinkers.
This will:

Reduce unnecessary duplication
The Service Model
The integrated service will provide:
1. Assertive Outreach (including Hospital Outreach)
2. Needs Assessment and Support Planning
3. Support to access longer-term placements and/ or specialist services where
appropriate

Outcome Based Commissioning (Year 2)
Outcome based commissioning:
•
Links financial reward to successful outcomes
•
Moves the focus to results achieved for individuals
•
Involves the overall setting of strategic goals i.e. Primary Outcomes
•
Provides greater clarity about what commissioners are trying to achieve
Outcome Based Commissioning
• Core contract with performance payment (80/20)
• Maximum payment is capped at 100% of the stated contract value
• A virtual period of 12 months to allow indicators and targets to be tested ahead
of financial implications being introduced in the second year

Considerations:
• Year 2 of the contract will introduce a results payment mechanism
• Intention to ring fence 20% of the contract value for this mechanism
• Achieving 70% of personalised outcomes attached to results payment

• Goliath & Mainstay IT Systems will be utilised for data collection
