Probation Outcome Form
This form should be used when providing information with regards to the outcomes of the
probationary period for all staff, where this applies. This information will be used to determine
the status of the employee; therefore it is essential that this is accurate and complete.


It is the managers’ responsibility to arrange and hold regular meetings to review and
manage performance during the probationary period
It is the managers’ responsibility to conduct a formal probationary review meeting and
complete this form before the end of the probationary period.

the end of the probationary period, in order to agree the outcome.
