Information you provide to Alberta Environment and Parks (AEP) is collected under the authority of Section 33(c) of the Freedom of Information and
Protection of Privacy (FOIP) Act and is managed in accordance with Part 2 of the FOIP Act. Your personal information is being collected for the purpose
of creating a WiN account and will be used for natural resource management and administration, which includes the enforcement of applicable laws.
The personal information provided will be managed by Active Network, an agent of the Crown. It will not be used or disclosed for any other purpose by
AEP without your consent, or unless required to do so by law. Should you wish to have your personal information removed, corrected, or have concerns

Consent to release information to Alberta Conservation Association (ACA)
The Alberta Conservation Association (ACA) works together with Alberta Fish and Wildlife to enhance natural resource management. Personal information
collected under this application is protected under Alberta’s Freedom of Information and Protection of Privacy Act. ACA is requesting access to your
personal information in order to provide you with services outlined below. Section 40(1) (d) allows disclosure of personal information if the individual the
information is about has identified the information and consented, in the prescribed manner, to the disclosure. This disclosure could include your name,
address, phone number and email address.

I consent to have Fish and Wildlife release my personal information, and any changes, to the ACA over the five (5) year
authorization period of my WIN card for the purpose of providing me information about ACA and where my levy dollars are being
spent. This may include receiving surveys asking for my input on the direction and priority areas for ACA.

I certify that I have successfully completed a Hunter Education Course in Alberta (or elsewhere). If you completed
a hunter education course in Alberta, you are required to enter the Hunter Certification Number provided to you by the
Alberta Hunter Education Instructor’s Association.

If you do not meet the criteria but wish to obtain hunting licences, please contact your District Fish and Wildlife Office.
Do you already have a WiN?
WiN card and associated WIN number are used to purchase both hunting and sportfishing licences and provides customers with
convenient access to their licensing information including their special licence priority information. If you know that you have a
WiN, please contact 1-888-944-5494 to obtain your existing WiN number. It may not only save you money but also ensures your
information is appropriately updated.
New WiN Card Customers
New WiN card customers must complete all sections on the first page
New WiN card customers planning to hunt must also complete Eligibility to Hunt sections
WiN Renewal Customers
IMPORTANT: Before leaving the Issuer renewal customers must:
a) Review their new licence document(s) for Information Accuracy
b) Have the Licence Issuer update any changes in their profile
c) Complete, sign and date all applicable sections of this application form
