What is it?

Baseline, Yearly or Triennial report for your district level school
wellness policy.

Can be easily shared with your stakeholders as it can be made
into a PDF file. The report could be shared at a School Board or
Parent Association Meeting

Post it on the School website or include the link in a school
newsletter.

Share it with the Montana Office of Public Instruction during the
Administrative Review (every 3 years) to verify your school
wellness policy implementation plan.

How to Complete it?

Use the same computer when completing the survey report. This
is an important step to remember.

Review the entire survey to become familiar with the information
requested in each section. You can advance the survey by
typing one character into each question.

Once you start entering data, you can save and update this file
as often as needed. You must press the SAVE button prior to
closing the document each time.

Once you’ve completed this report to your satisfaction, press the
SAVE Button and save a PDF file before pressing the SUBMIT
button.

Press the SUBMIT button. Once submitted, we can also send the
report back to you in Adobe Acrobat (PDF) format too.

Final Steps

Make a PDF File of it and share with your School Wellness
Committee and Administration.

Revise the plan as needed and press the SAVE Button.

If you want to share it with the Team Nutrition Program, press the
SUBMIT Button. It can be used for your Administrative Review
with the Office of Public Instruction which occurs every 3 years.

Share the plan with your stakeholders by posting it on your
website.

Keep a copy on file for documentation of your implementation
and evaluation plans for the School Wellness Policy
