All participants must bring their own ball and water bottle. Shin guards and appropriate soccer shoes are
required equipment for tryout.

Registration will be on-site the day of tryouts. Please arrive 20 minutes before scheduled tryout time.

If you do not print the form in advance, you will need to provide the following on the night of
tryouts: Player’s name, birth date, address, email, phone number and parent signature

In accordance with Iowa Soccer Association rules and policies, U11-U18 players that make teams will
receive an invite after tryouts via a phone call, email or our website. Information regarding team acceptance
will be distributed at tryouts.

Club, I/we hereby waive any legal claims and release the Muscatine Soccer Club, its Board of Directors, Employees and
the team on which my son/daughter is participating from any claim of negligence. I understand the participation in soccer
may cause injury and I agree not to file any claim against Muscatine Soccer Club, and it’s Board of Directors, Employees
or the team in the event of injury to my son/daughter while participating in this soccer program. I hereby give my consent
in the event of injury to have an athletic trainer, medical doctor, nurse, hospital, clinic or other qualified health care
provider, give medical assistance and/or treatment, and agree to be financially responsible for the cost of this assistance
